地震伤亡时间曲线在医疗救援中的意义

摘要
目的探讨地震发生后累计伤亡人数与时间的关系,寻求医疗救援反应评价指标。方法统计震后第1~17天累计地震伤亡人数,将地震总累计死亡人数与震后第3天累计死亡人数之比作为总医疗救援反应指数(total medical rescue index,TRI)。将地震发生后每天累计死亡人数与2天前累计死亡人数之比作为医疗救援反应预警指数(prewarning medical rescue index,PRI),再将每天医疗救援反应预警指数进行自身比较。结果地震发生后头5天死亡28 881人,占总死亡人数的42.40%,受伤193 347人,占总受伤的54.41%。震后5~13天,伤亡人数增加速度有所减缓,13天后,伤亡人数增加不明显。总医疗救援反应指数为4.470。医疗救援反应预警指数最大2.1,且出现在地震后72小时,前7天均>1.4,7天以后医疗救援预警指数逐渐下降。结论总医疗救援反应指数和医疗救援反应预警指数是评价医疗救援反应速度和效率的客观指标,总医疗救援反应指数越大,地震医疗救援反应速度和效率越低。医疗救援反应预警指数对地震救援期间决定医疗救援力量的调整有一定的指导意义。
Objective To investigate the relationship between the cumulative number of casualties and the time after the earthquake,and to seek the evaluation index of medical rescue response.Methods The number of cumulative casualties was analyzed from the 1st day to 17th day after the earthquake,and the ratio of the total cumulative death toll to the number within the first 3 days was the total medical rescue index(TRI).The ratio of everyday cumulative death toll to the cumulative death toll within the past 2 days was the prewarning medical rescue index(PRI),and then self-comparison was taken in everyday PRI.Results Totally 28.881 thousand died in the first 5 days,accounting for 42.40% of the total number of deaths,193.347 thousand injured,54.41 percent of the total injuries.The rate of increasing casualties slowed down from the 5th day to the 13th day,and the increased number of casualties was not obvious after 13 days,with the TRI of 4.470;the maximum PRI was 2.1 appearing at 72 hour after the earthquake,which was more than 1.4 in the previous 7 days,then began to decline.Conclusion TRI and WRI are the objective indicators to evaluate the speed and efficiency of the medical rescue response.Bigger TRI results in lower speed and less efficiency of the medical rescue response.PRI could be useful for adjusting medical rescue strength during earthquake medical rescue.
