B超在地震灾害颅脑损伤术中及术后的应用

摘要
目的探讨B超在地震灾害颅脑损伤术中及术后的应用价值。方法应用B型超声仪,对术中急性脑膨出,怀疑有多发性颅内血肿者实施检查,术后经颅骨缺损或颅骨钻孔处进行水平、冠状或滑行扫查。结果58例地震灾害开颅手术患者,术中进行B超检查24例,发现不同部位的颅内血肿或积血18例,同侧颅内血肿2例,B超引导下手术清除血肿14例;术后28例同时进行CT和B超检查,各发现血肿6例和5例,脑水肿及脑挫裂伤23例和3例。结论对地震灾害颅脑损伤患者,术中及术后B超检查能快速、准确地发现残留血肿与迟发血肿,实时指导医生手术和治疗方案调整,缩短检查时间,明显提高救治成功率,降低残废率。
Objective To investigate the value of application intraoperative and postoperative B-type ultrasonic for craniocerebral injury patients suffering from earthquake.Methods The patient who were considered multiple intracranial hematomas for acute craniocele in surgery were checked by B-type ultrasonic.After operation,Checking the patients with B-type ultrasonic through part of skull defects or hole for level,coronalis,and skidding scan.Results 58 craniocerebral patients suffering from earthquake were operated.24 cases were checked intraoperatively.18 cases were intracranial hematomas of different parts,2 cases were intracranial hematomas of same side.14 cases were operated by guide of B-type ultrasonic.28 cases were checked by CT and B-type ultrasonic after operation simultaneously.The results checked by CT were the same as B-type ultrasonic.6 cases of hematoma,23 cases of edema and cerebral contusion were found by CT.5 cases of hematoma,3 cases of edema and cerebral contusion were found by B-type ultrasonic.Conclusion Multipe intranial hematomas were accurately oriented by B-type ultrasonic in operation.It has significant meaning for recognition of craniocele intraoperation.Late hematomas can be accurately checked through skull defects after operation.In the earthquake disaster,the ultrasound applied after the head injury could promptly and accurately instruct doctor to perform the surgery,increase success ratio,reduces the disability rate.
