143例地震伤的影像诊断特点

摘要
回顾性分析143例地震伤的影像学及临床资料,以探讨地震伤的影像诊断特点。143例患者均行X线平片检查,78例行CT检查,19例行MRI检查。四肢骨折81例,单发骨折34例,多发骨折47例,开放性骨折6例,闭合性骨折75例。关节脱位14例,软组织内异物5例,膝关节半月板损伤1例,气性坏疽1例。脑硬膜外血肿4例,颅内血肿2例,脑挫裂伤9例,肾挫伤2例、肝挫伤1例。22例脊椎地震伤患者中,脊椎压缩骨折20例,爆裂性骨折2例。单发椎体骨折12例,多发椎体骨折10例,共累及31个椎体。31个椎体骨折中,颈椎3个,胸椎12个,腰椎14个,骶椎2个。30例胸部地震伤均为复合伤,其中肋骨骨折18例,胸椎骨折8例,肩胛骨骨折3例,锁骨骨折3例,胸骨骨折1例,胸壁软组织伤5例,肺挫裂伤5例,肺内血肿1例,肺部感染4例,气胸及液气胸6例,双侧胸腔积液1例。
To explore the imaging features of the earthquake-related injuries,143 patients who survived the earthquake but suffered from the injuries were retrospectively analyzed.All patients underwent X-ray plain film examination,some of whom were performed CT(n=78) and/or MRI(n=19).Among the 81 cases of limb bone fracture,there were the single-level fracture(n=34),multiple-level fracture(n=47),compound fracture(n=6),and closed fracture(n=75),dislocation of joint(n=14),foreign body in soft tissue(n=5),meniscus of knee joint(n=1),gaseous gangrene(n=1),epidural hematoma(n=4),intracranial hematoma(n=2),contusion and laceration of brain(n=9),contusion of kidney(n=2) and contusion of liver(n=1).In the 22 cases of the earthquake-related vertebral injury,there were the vertebral compression fracture(n=20),vertebral bursting fracture(n=2),single-level vertebral fracture(n=12),and multiple-level vertebral fracture(n=10).Among the 31 vertebral bodies of fracture,the fracture of ceruical vertebra,thoracic vertebra,lumbar vertebra and sacral vertebra was found in 3,12,14,2 vertebral bodies,respectively.All of the 30 cases of the earthquake-related thoracic injury were the combined injury,including the fracture of rib(n=18),thoracic vertebral fracture(n=8),fracture of shoulder blade(n=3),fracture of clavicle(n=3),fracture of sternum(n=1),soft tissue injury of chest wall(n=5),contusion of lung(n=5),hematoma of lung(n=1),pulmonary infection(n=4),pneumatothorax and hydropneumothorax(n=6) and pleural effussion(n=1).
