脾脏转移性肿瘤的影像学特征探讨与实验研究

摘要
研究目的
     1.收集南方医院近14年来的64例脾脏转移瘤的影像学资料并结合国内外相关文献,分析其影像学特点,探讨脾脏转移瘤的影像学诊断与鉴别诊断价值。
     2.建立大鼠脾脏VX2肿瘤模型,研究大鼠脾脏VX2肿瘤磁共振成像(Magnetic Resonance Imaging,MRI)表现于病理改变之间的关系。分析大鼠脾VX2肿瘤超顺磁性氧化铁(SPIO)增强MR图像上信号强度变化与病理学改变之间的关系。探讨超顺磁性氧化铁(SPIO)在大鼠脾脏占位性病变中的应用价值。
     材料与方法
     1.研究对象
     1.1 回顾1992.1-2005.12南方医院B超、CT和MRI检查发现脾脏转移性肿瘤64例,30例经穿刺或手术后经组织病理学证实,所有病例均经临床确诊。
     1.2 动物模型制作 将20只清洁级SD大鼠以3%戊巴比妥钠(0.2~0.3ml/只)麻醉后固定于木板上,腹部被皮、常规消毒后剖开腹腔,直视下用16号粗针头成锐角刺入脾实质,尔后将大小约2×2mm瘤组织块植入,缝合腹部肌肉、皮肤。9~14天左右将荷肿瘤鼠行MRI扫描。5只清洁级SD大鼠作为正常对照组。所有大鼠均由南方医院实验动物中心提供。
     2.主要仪器设备及试剂
     采用德国西门子Magnetom vision plus 4超导型全身磁共振扫描仪,场强1.5Tesla,采用颈部正交线圈。在图像存档和传输系统(PACS)工作站上进行图像分析及数据测量工作。超顺磁性氧化铁(Superparamagnetic iron oxide,
Objective
    1. To collect the imaging appearance of splenic metastasis in 64 patients at Nanfang hospital during lastest 14 years and integrate related literatures. In order to analyze the imaging characteristic of splenic metastasis, and discuss the value of diagnosis and differentiated diagnosis in splenic metastasis.
    2. A rodent model of VX2 tumor to spleen was used to study the relationship between
    pathologic change of splenic carcinoma and magnetic resonance imaging(MRI). To analyze relationship between the change of the signal intensity on superparamagnetic iron oxide(SPIO) enhanced MR images and phagocyte. To discuss the application value of SPIO enhanced detection in splenic focal lesion of rat.
    Materials and Methods
    1. Objects in study
    1.1 To study the splenic metastases in 64 patients which were diagnosed at ultrasonography(US) and computer tomographty(CT) and MRI examinations at Nanfang hospital during the yearsl992.1-2005.12 retrospectively. 30 patients were confirmed by histopathology after splenectomy and fine-needle biopsy.
