小青龙汤加减治疗支气管哮喘发作期的理论及临床研究
摘要
通过对张仲景《伤寒杂病论》及历代医家有关哮喘论述的深入研
究，结合导师和个人的多年临床体会，认为肾阳亏虚、寒饮伏肺是哮
喘反复发作的宿根；外感、七情、食伤是哮喘发作的诱因；寒饮遇感
触动之后，出现寒饮化热、饮热夹杂，是哮喘发作期的主要证候特点。
据此，我们提出哮喘发作期治疗应温清并用，肺肾同治，以温肺化饮、
补肾纳气、清热宣肺、降逆涤痰法作为哮喘发作期的治疗大法。组方
以小青龙汤为主加减。
通过临床研究发现，本方具有良好的平喘、化痰、止咳功效，并
对改善气道功能、动脉血PaCO_2、PaO_2有明显作用，说明温肺化饮、
补肾纳气、清热宣肺、降逆涤痰法作为哮喘发作期的治疗大法是有临
床依据的，也验证了我们对哮喘发作期病机认识的正确性。
Through thoroughly research about dissertation of asthma of ShangHanLun and
bypast doctors, and combining with my clinical experience, we consider that asthenic
nephroyang ,cold phlegm in lung are essential reasons of its outbreak, and infection,
psychic obstacle, dietetic intemperance are its inducements. Our viewpoint is that the
pathology characteristic of asthma during its outbreak is cold phlegm generating heat,
phlegm heat missing. According to the above reason, we put forward that the main
therapy rule should be warming and cooling, treating both lung and kidney during the
outbreak stage of asthma.
Through clinic research, we find it can effectively relieve cough, eliminate phlegm,
improving the function of airway, and regulate PaCO2 and Pa02. The result shows that
warming lung and eliminate yin, replenishing the inborn qI, controlling and promoting
inspiration, cooling heat and activating the flow of lung qi, cleaning the phlegm and
sending down the qi of lung are the main rule of treatment which is based on the clinic
practice. It also confirmed our cognition about the outbreak mechanism of asthma.
