超声在评估肝爆震伤中的应用价值

摘要
目的探讨肝爆震伤后肝脏常规超声的应用价值。方法制作肝脏轻度、中度、重度爆震伤动物模型,于爆震前、爆震后即刻行肝脏及腹腔常规超声扫查,并与肝脏病理以及爆震前、后肝丙氨酸氨基转移酶(Alanine aminotransferase,ALT)改变进行对比分析。结果 1、伤后即刻常规超声检查轻度爆震伤组肝未见明显异常声像图表现;中、重度爆震伤组除可见不同程度的肝包膜断裂、包膜下血肿、明显的肝实质连续性中断、中断的肝实质间积液、腹腔内积液等一系列肝损伤的表现外,绝大例数见一处或多处肝实质局限性不均匀稍增强,以重度爆震伤组明显。2、三组爆震后即刻肝脏ALT均较爆震前增大,爆震前、爆震后即刻各组肝脏ALT自身对照比较(P<0.05)有统计学意义,爆震后即刻三组肝脏ALT组间以及任意两两比较(P<0.05)有统计学意义。结论爆震伤后肝脏超声声像图呈现复杂性、多样性的特点,常规超声可以实时评估爆震伤后肝脏形态和结构改变,起到快速筛选急危重患者的重要作用,具有重要的应用价值。
Objective To investigate the liver conventional ultrasonoscopy characteristics and values of different degree of liver blast injury. Methods: Building liver mild, moderate and severe blast injury animal models, the liver and abdominal conventional ultrasound scan and contrastive analysis of the liver pathology and the changes of hepatic Alanine aminotransferase(ALT) were proceeded before the detonation and after detonation immediately. Results: 1. The immediately routine ultrasound examination of the mild injury group after injury had found no obvious ultrasonogram; while with the exception of different degree liver capsular rupture, subcapsular hematoma, obvious liver parenchyma interruption, effusion between the interrupt liver parenchyma, and abdominal cavity effusion etc. a series of liver injury were found in the moderate and severe group, Most cases of liver parenchyma or multiple limitations uneven slightly enhanced to severe blast injuries significantly. 2. The immediately liver ALT value of the three groups after detonation were higher than before detonation, each group's liver ALT value comparing before and after detonation immediately had statistically significance(P < 0.05), and the three groups' and any two groups' liver ALT value comparing all had statistically significance(P < 0.05). Conclusions: After the blast injury of liver sonographic presentation complexity, diversity characteristics, Conventional ultrasound can real-time assessment liver morphology and structural changes after blast injury, and which plays a critical role in rapid screening critically ill patients.
