口腔颌面部间隙感染治疗效果的回顾性分析

摘要

目的回顾性分析口腔颌面部间隙感染的临床特征、治疗方法以及治疗效果,为临床应用提供参考价值。方法2014年3月—2018年9月间,在该院方便选取106例口腔颌面部间隙感染患者的作为研究对象进行回顾性分析,分析的内容主要包括感染部位及感染源、临床特征、治疗方法及治疗效果,并给予针对性的预防措施和治疗建议。结果全部106例口腔颌面部间隙感染患者在接受治疗后,其中有103例患者的病情痊愈,所占比例为97.17%,剩余3例患者并发了败血症和脓毒血症,其中有2例的患者的感染源为蜂窝织炎,1例患者的感染源为颌下间隙感染。死亡病例有2例,其中有1例患者的死亡原因为口底多间隙感染并发感染性休克,全身多器官功能衰竭,离院后死亡;1例车祸受伤后1周伴颊部感染,入院后并发颅内感染转ICU,治疗无好转,离院后死亡;而在治疗时间方面,当口腔颌面部为单间隙的感染患者时,其治疗时间明显短于多间隙的感染患者,其中单间隙感染患者的治疗时间介于4～5 d,平均治疗时间为(5.3±2.2)d,多间隙感染患者的治疗时间介于7～11 d,平均治疗时间为(9.1±2.3)d。结论由于口腔颌面部间隙感染发生时其病情较急,所以多采用综合治疗,比如全身抗炎、局部病变以及营养支持等,且有较好的治疗效果。
        Objective To retrospectively analyze the clinical features, treatment methods and therapeutic effects of oral and maxillofacial space infections, and to provide reference for clinical application. Methods From March 2014 to September2018, 106 patients with oral and maxillofacial space infections in our hospital were convenient selected as the study subjects. The contents of the analysis included infection site, infection source, clinical characteristics, treatment methods and treatment effect, and given targeted preventive measures and treatment recommendations. Results Among 106 cases of oral and maxillofacial space infection, 103 cases were cured, accounting for 97.17%. The remaining 3 cases were complicated with sepsis and sepsis. Cellulitis was the source of infection in 2 cases and submandibular space infection in 1 case. There were 2 deaths, one of which was caused by infection of multiple spaces on the floor of the mouth complicated with septic shock, multiple organ failure of the whole body, and death after leaving the hospital. The treatment time of the patients with single space infection was significantly shorter than that of the patients with multiple space infection. The treatment time of the patients with single space infection was between 4-5 d, the average treatment time was(5.3±2.2)d, the treatment time of the patients with multiple space infection was between 7 d and 11 d, and the average treatment time was(9.1 ±2.3)d.Conclusion As oral and maxillofacial space infections occur when the disease is more acute, so the use of comprehensive treatment, such as systemic anti-inflammatory, local lesions and nutritional support, and has a better therapeutic effect.
