深圳市龙岗区空肠弯曲菌感染状况及耐药性分析

摘要
目的了解深圳市龙岗区腹泻人群中空肠弯曲菌的感染状况及其耐药性。方法收集2016年9月-2017年3月深圳市龙岗区某医院腹泻患者粪便或肛拭子样品和来自龙岗中心城养鸡场、农贸市场、超市等地的活禽与肉类等样品进行空肠弯曲菌分离培养和鉴定,并对分离得到的菌株进行耐药性分析。结果 51份腹泻患者样品分离出6株空肠弯曲菌,阳性率为11.76%,全部阳性样品均来自11岁以下儿童。60份食品和外环境的样品分离出21株空肠弯曲菌,阳性率为35.00%,其中来自农贸市场的阳性率为57.14%(8/14),来自不同超市的阳性率为25.58%(11/43)。空肠弯曲菌对萘啶酸、环丙沙星、四环素的耐药率在90%以上,食品来源和人源的阳性菌株对萘啶酸、环丙沙星或四环素全部耐药。人源菌株对5种抗生素产生耐药的占16.67%。食品来源菌株中有47.62%对6种抗生素产生耐药作用,其中有9.52%的菌株对11种抗生素全部耐药。结论深圳市龙岗区空肠弯曲菌感染率较高,所分离的菌株耐药现象较为严重且均具有多重耐药性,人源和食品来源的耐药谱相一致,应对抗生素在临床治疗和禽畜类养殖中的使用进行长期系统的监测,并加强用药管理。
        Objective To understand the infection status and drug resistance of Campylobacter jejuni( C.jejuni) in patients with diarrhea in Longgang district of Shenzhen. Methods C. jejuni was isolated and cultured from stool and anal swab samples of diarrhea patients from a hospital in Longgang district of Shenzhen during September, 2016 to March, 2017 and samples of poultry and livestock from chickens, farmers' markets and supermarkets. And then the drug susceptibility test was carried out for the isolated strain. Results Six C. jejuni isolates were isolated from 51 diarrhea patients samples; the isolation rate was 11.76%. All positive samples were from children under 11 years old. 21 C. jejuni isolates were isolated from 60 food and outer environment samples; the isolation rate was 35.08%. The isolation rate from the farmer's market was57.14%( 8/14), and the isolation rate from different supermarkets was 25.58%( 11/43). The resistance of C. jejuni to naphthenic acid, ciprofloxacin and tetracycline was over 90%, and the isolates from food sources and human sources were all resistant to naphthidine, ciprofloxacin or tetracycline. The resistance of human strains to 5 antibiotics was 16.67%. Of the isolates from food source, 47.62% were resistant to 6 antibiotics, of which 9.52% were resistant to all 11 antibiotics.Conclusions The C. jejuni infection rate was high in diarrhea patients in Longgang district of Shenzhen. The antibiotic resistance of isolated strains was quite serious and some isolates showed multiple drug resistance. The drug resistance spectrum of the isolates obtain from human and food sources was consistent, and long-term systematic monitoring of antibiotic use in clinical treatment and poultry farming should be carried out, and drug management should be strengthened.
