摘要
目的探讨大面积脑梗死不同阶段头颅CT和MRI的影像特点。方法选择2012年1月至2018年12月在我院接受治疗的60例大面积脑梗死患者为研究对象,分别给予头颅MRI与CT检查,对比两组患者的检查结果,分析大面积脑梗死不同阶段头颅CT和MRI的影像学特点。结果 (1)大面积脑梗死患者经CT、MRI检查对比,CT诊断总检出率为71.7%;MRI总检出率为96.7%;组间对比,MRI检出率显著高于CT检出率,P<0.05;(2)两种检查的影像学特点,CT检查:经CT检查可见,患者发病后10~12h,颅内组织均未见明显变化,24h后可见边界清晰的低密度灶,急性期患者可见病灶区低密度,同时皮髓质密度无变化,且边界清晰。MRI检查:急性期患者MRI影像表现可见病灶T_1加权像呈低信号,T_2加权像呈稍高信号,亚急性期病灶T_1加权像呈低信号,T_2加权像呈明显均匀高信号;慢性期病灶T_1加权像呈低信号、T_2加权像均高信号。结论大面积脑梗死患者,MRI检查更加理想,更有价值,值得推广。
        Objective To investigate the clinical and imaging features of CT and MRI in different stages of larges-cale cerebral infarction. Methods Sixty patients with large-area cerebral infarction who underwent treatment from January 2012 to December 2018 were enrolled in the study. 60 patients were given MRI and CT examinations. The clinical results of the two groups were compared. To analyze the diagnostic and imaging features of CT and MRI in different stages of large-scale cerebral infarction. Results(1) Comparisons of CT and MRI in patients with massive cerebral infarction: the total detection rate of CT diagnosis was 71.7%, the total detection rate of MRI was 96.7%, and the detection rate of MRI was significantly higher than that of CT, P<0.05;(2) The imaging features of two kinds of examinations: CT examination showed that there was no obvious change in the intracranial tissues within 10~12 hours after the onset of the disease, low density lesions with clear boundary could be seen after 24 hours, low density lesions could be seen in the acute phase, while the density of cortex and medulla had no change, and the boundary was clear.MR imaging: In acute stage, T_1-weighted image showed low signal, T_2-weighted image showed slightly high signal, T_1-weighted image showed low signal, T_2-weighted image showed obvious uniform high signal, and T_1-weighted image showed low signal and T_2-weighted image showed high signal in chronic stage. Conclusion For patients with largearea cerebral infarction, the MRI diagnosis method is more ideal and worth promoting.
