二维斑点追踪技术测定青海果洛地区人群右室整体收缩功能的可行性及准确性分析

摘要
目的探讨二维斑点追踪技术(STI)在青海果洛地区人群右室整体收缩功能检测的可行性及准确性。方法选择2016年5月至2018年2月青海果洛高原地区(在上海体检)右心功能减低人群患者40例作为对象,设为观察组;选择同期体检的果洛正常人群40例,设为对照1组;选择上海地区右心功能正常人群40例,设为对照2组。采集STI技术测量并获得心尖四腔二维图像及三维全容积动态图像,将其输入QLAB 10.8工作站,获取右室游离壁三尖瓣环收缩期峰值位移(T1)、室间隔三尖瓣环收缩期峰值位移(T2)、三尖瓣环连线中点收缩期峰值位移(Tm)及右室收缩功能(RVEF)水平;将三维全容积动态图像传输到TOMTEC工作站中完成左心室射血分数(LVEF)与右心室射血分数(RVEF)计算;采用Pearson相关性分析软件对三尖瓣位移参数与RVEF进行相关性分析。结果观察组T1、T2、Tm及RVEF水平,均低于对照1组与对照2组,差异有统计学意义(P<0.05);观察组LVEDV、RVEDV、LVEF及RVEF水平均低于对照1组和对照2组,差异有统计学意义(P<0.05);观察组LVESV、R VESV水平高于对照1组和对照2组,差异有统计学意义(P<0.05);SPSS Pearson相关性分析结果表明:青海果洛地区人群RVEF与T1、T2、Tm呈正相关性(P<0.05)。结论 STI技术用于青海果洛地区人群右室整体收缩功能测定中是可行的,能准确的评价右心功能减低人群心功能水平。
        Objective To explore the feasibility and accuracy of two-dimensional speckle tracking technology(STI for detecting right ventricular systolic function among people of Guoluo,Qinghai province.Me thods A total of 40 patients with right heart function reduction in Guoluo Plateau region of Qinghai province who underwent physical examination in Shanghai from May 2016 to February 2018 were selected as the observation group.40 healthy people who underwent physical examination were selected as the control group 1 and 40 people with normal right heart function in Shanghai were selected as the control group 2.STI technique was used to measure and obtain the apical four-chamber two-dimensional image and the three-dimensional full-volume dynamic image and they were input into the QLAB10.8 workstation to obtain the right ventricular free wall tricuspid annulus systolic peak displacement(T1),ventricular septal tricuspid annulus peak systolic displacement(T2),midpoint systolic peak displacement(Tm) and right ventricular systolic function(RVEF) levels;3 D full-volume dynamic images were transmitted to TOMTEC workstation to calculate left ventricular ejection fraction(LVEF) and right ventricular ejection fraction(RVEF).Correlation analysis between tricuspid valve displacement parameters and RVEF was performed using SPSS Pearson correlation analysis software.Re-s ults The levels of T1,T2,Tm and RVEF in the observation group were lower than those in the control group 1 and the control group 2 and the difference showed statistical significance( P <0.05).The levels of LVEDV,RVEDV,LVEF and RVEF in the observation group were lower than those in the control group 1 and the control group 2 and the difference showed statistical significance(P <0.05).The levels of LVESV and RVESV in the observation group were higher than those in the control group 1 and the control group 2 and the difference showed statistical significance(P <0.05).The SPSS Pearson correlation analysis showed that the RVEF of people in Guoluo,Qinghai province was positively correlated with T1,T2 and Tm and the difference showed statistical significance( P <0.05).Conclus ion STI technique is feasible for the determination of right ventricular systolic function in the population of Guoluo,Qinghai province and can accurately evaluate the cardiac function of people with right heart function reduction.
