摘要

目的调查糖尿病肾病透析患者的自我感受负担情况,分析相关影响因素。方法采用回顾性研究方法,选择2014年3月至2016年10月在医院进行长期性血液透析的糖尿病肾病患者140例为观察组,选择同期诊治的糖尿病肾病非透析患者140例为对照组,对2组患者进行一般情况与自我感受负担的调查与相关性分析。结果观察组的生理症状、心理症状、困扰指数与总分评分明显高于对照组,差异均有统计学意义(P<0.05)。单因素条件logistic回归分析结果显示透析时间、婚姻状况、高血压、病程与自我感受负担总分密切相关(P<0.05);条件性Logistic回归法结果显示透析时间、高血压、病程为影响自我感受负担总分的主要危险因素(P<0.05)。结论糖尿病肾病透析患者多存在自我感受负担加重情况,透析时间、高血压、病程为主要的影响因素,在透析期间要积极加强个性化健康指导干预。
        Objective To investigate the self perceived burden of patients with diabetic nephropathy(DN),and to analyze the influencing factors.Methods A retrospective study method was used,a total of 140 patients with DN who underwent long-term hemodialysis in our hospital from March 2014 to October 2016 were enrolled as observation group,at the same time, the other 140 patients with DN who did not receive maintenance hemodialysis were enrolled as control group.The general information and self perceived burden were observed and compared between the two groups.Results The physiological symptoms,psychological symptoms,distress index and total scores in observation group were significantly higher than those in control group(P<0.05).Single factor conditional Logistic regression analysis showed that the dialysis duration,marital status,hypertension,course of disease were closely related with the self perceived burden scores(P<0.05). The conditional Logistic regression analysis showed that the dialysis duration, hypertension,course of disease were the main risk factors influencing the self perceived burden scores(P<0.05).Conclusion The aggravation of self perceived burden exists in the patients with DN undergoing maintenance hemodialysis.and the dialysis duration, hypertension,course of disease are the main risk factors,thus, it is necessary to actively strengthen the personalized health guidance intervention during hemodialysis.
