摘要
目的分析阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者QT间期及其离散度,探讨其在OSAHS病情评估中的价值。方法回顾性分析82例OSAHS患者(观察组)及79例单纯鼾症者(对照组)的资料,记录并比较两组的年龄、性别、体重指数(BMI)、血压、心电图及多导睡眠图相关数据。结果观察组的呼吸暂停低通气指数(AHI)、 QT间期离散度(QTd)及校正QT间期离散度(QTcd)均高于对照组(P <0.05)。观察组的LSaO2、 MSaO2均低于对照组(P <0.05)。两组的年龄、性别、血压、 BMI、 QT间期、校正QT间期(QTc)比较均无统计学差异(P>0.05)。相关性分析显示OSAHS患者的QTd、QTcd与AHI呈正相关,与MSaO2呈负相关; QTcd与LSaO2呈负相关(P均<0.05)。校正年龄、 BMI、 SBP、 DBP等因素后,偏相关分析显示OSAHS患者的QTd、 QTcd与AHI呈正相关,与MSaO2呈负相关(P均<0.05)。结论 OSAHS患者心电图QTd、QTcd增高,提示发生室性心律失常的风险增加。
Objective To analyze the QT interval and QT interval dispersion in patients with obstructive sleep apnea hypopnea syndrome(OSAHS), and to explore the value in OSAHS disease assessment. Methods The data of 82 cases of OSAHS patients(observation group) and 79 cases of patients with simple snoring(control group) were analyzed retrospectively. The age, gender, body mass index(BMI),blood pressure, electrocardiogram and polysomnography were collected and compared between the two groups. Results The apnea hypopnea index(AHI), QT interval dispersion(QTd) and QT interval corrected dispersion(QTcd) of the observation group were higher than those of the control group(P <0.05). The LSaO2 and MSaO2of the observation group were lower than those of the control group(P <0.05).No statistical difference was found in the age, gender, blood pressure, BMI, QT interval and corrected QT interval(QTc)(P >0.05).Correlation analysis showed that OSAHS patients' QTd and QTcd were positively correlated with AHI and negatively correlated with MSaO2(P <0.05), and QTcd was negatively correlated with LSaO2(P <0.05). After correcting factors such as age, BMI, SBP and DBP, partial correlation analysis indicated that QTd and QTcd were positively correlated with AHI and negatively correlated with MSaO2(P <0.05).Conclusions Increased QTd and QTcd in OSAHS patients indicates the increased risk of ventricular arrhythmia.
