PVP和PKP治疗老年骨质疏松性骨折的临床疗效分析

摘要

目的探讨椎体成形术(PVP)和椎体后凸成形术(PKP)治疗老年骨质疏松性骨折的临床疗效。方法选取本院2016年4月~2017年4月期间收治的62例老年骨质疏松椎体压缩性骨折患者为研究对象。根据不同手术治疗方法,将患者分为PVP组和PKP组,各31例。比较两组患者骨水泥注入量、手术时间、住院时间以及骨水泥渗漏情况,比较患者术前、术后疼痛程度、背部功能及椎体前缘高度情况。结果两组患者骨水泥注入量、住院时间以及骨水泥渗漏情况比较比较差异无统计学意义;PKP组手术时间明显长于PVP组(P<0.05);相较于术前,两组患者术后VAS评分、ODI评分均显著降低(P<0.05),椎体前缘高度显著升高(P<0.05);PKP组患者术后椎体前缘高度明显高于PVP组(P<0.05),其余各时段VAS评分、ODI评分及椎体前缘高度组间比较差异无统计学意义。结论 PVP和PKP均能快速有效的缓解老年骨质疏松椎体压缩性骨折患者的疼痛症状、改善活动功能,临床疗效显著,均具有重要的临床应用价值。
        Objective To investigate the clinical efficacy of vertebroplasty(PVP) and kyphoplasty(PKP) in the treatment of osteoporotic fractures in the elderly. Methods 62 elderly osteoporotic vertebral compression fractures were selected from April 2016 to April 2017 in our department of orthopedics. According to different surgical treatment methods, the patients were divided into PVP group and PKP group, 31 cases in each group.The amount of bone cement injection, the time of operation, the time of hospitalization and the leakage of bone cement were compared between the two groups, and the degree of pain, back function and the height of the anterior edge of the vertebral body were compared between the patients before and after the operation. Results There was no significant difference in the amount of bone cement injection, the time of hospitalization and the leakage of bone cement in the two groups. The operation time in the group PKP was significantly longer than that in the PVP group(P<0.05). Compared with the preoperative, the VAS score and the ODI score of the two groups were significantly decreased(P<0.05), and the height of the anterior margin of the vertebral body increased significantly(P<0.05); in group PKP, the height of anterior edge of vertebral body was significantly higher than that of group PVP(P<0.05). There was no statistical difference between the other periods of VAS score, ODI score and the height of anterior vertebral height in the vertebral body. Conclusion Both PVP and PKP can quickly and effectively alleviate the pain symptoms and improve the function of the patients with osteoporotic osteoporotic vertebral compression fracture. The clinical effect is significant, and all of them have important clinical value.
