摘要
目的通过2例肺结核合并HIV抗体不确定样本的跟踪检测,探讨对感染合并不确定样本进行核酸检测的必要性,为检测策略的完善和应用推广提供依据。方法对2例肺结核合并HIV抗体不确定者跟踪检测,在首次、4周、8周分别进行3次HIV抗体确证检测,在第8周同时进行病毒核酸检测。结果病例1:3次HIV抗体确证试验,带型均不能判定为阳性,第3次同时进行病毒核酸定性为阴性,判定为HIV-1抗体阴性。病例2:3次HIV抗体确证试验,带型在第3次出现进展,核酸定性阳性,核酸定量12 000 copy/ml,判定为HIV-1抗体阳性。结论对感染合并HIV抗体不确定样本,进行核酸检测进一步明确诊断非常必要。
        Objective Through tracking and detecting 2 case of uncertain antibodies in pulmonary tuberculosis complicated with HIV,to explore the necessity of nucleic acid detection for uncertain samples with infection so as to provide basis for improving detection strategy and application. Methods Following up detection and tracking was performed on the two uncertain antibodies with pulmonary tuberculosis infection complicated with HIV. 3 HIV antibody confirmatory tests were performed for the first time,4 weeks,and 8 weeks,and viral nucleic acid was detected at the eighth week. Results Case one underwent 3 HIV antibody confirmatory tests,belt type could not be determined as positive,qualitative and quantitative detection of viral nucleic acid was performed at the third time,without response and TND,and diagnozed as negative for HIV-1 antibodies. Case two underwent3 HIV antibody confirmatory tests,the belt type got progressed in the third time,and the nucleic acid was positive and the nucleic acid was 12 000 copy/ml. The diagnosis was positive for HIV-1 antibodies. Conclusion It is necessary to combine nucleic acid detection to make a further diagnosis for the infection with HIV antibody uncertainty samples.
