2012-2017年四川省50岁及以上艾滋病病毒感染者/艾滋病患者流行特征分析

摘要
目的了解四川省50岁及以上人群HIV感染特征,为制定针对该人群的艾滋病干预措施及防治策略提供科学依据。方法通过艾滋病防治基本信息系统收集2012-2017年四川省50岁及以上HIV/AIDS病例资料,对病例人口学特征及感染途径进行分析。结果 2012-2017年四川省共报告50岁及以上HIV/AIDS病例35 625例,占总报告病例数的33.51%,50岁及以上病例占当年报告总数比例呈现逐年上升趋势。该人群病例以男性为主,男女性别比为2.78:1,年龄分布主要为50～69岁(78.46%),已婚有配偶占57.27%,小学及以下文化程度占69.25%,职业以农民或民工为主(69.51%),绝大多数为汉族(96.51%)。感染途径以异性传播为主(96.02%),男女性异性传播构成比分别为95.30%和98.00%。阳性样本检测来源以术前/受血前/其他就诊者检测为主(68.96%),首次CD4细胞检测结果以小于350个/ml为主(67.56%)。结论四川省50岁及以上人群HIV感染绝对数及其占新报告病例总数的比例增长迅速,已成为艾滋病感染的重点人群,应针对该人群的特征制定综合性的艾滋病防控策略。
Objective To understand the epidemiological characteristics of HIV/AIDS patients aged 50 years or older and provide scientific evidence for developing intervention measures and control strategies for this population. Methods Data on HIV/AIDS patients aged 50 years or older in Sichuan province from 2012 to 2017 were collected through the HIV/AIDS Prevention and Control Information System.Sociodemographic characteristics and transmission route of patients 50 years or older with HIV/AIDS(PLHIV) were analyzed. Results Between 2012 and 2017, 35 625 HIV positive people aged 50 years or above were identified in Sichuan province,accounting for 33.51%of total newly-reported cases. The proportion of PLHIV patients aged 50 years or more rose year by year. The ratio between male and female cases was 2.78∶1. Patients aged 50 to 69 years old accounted for 78.46% of newly reported cases. 57.27% of elderly patients were married and 69.25% had a primary school education level or below. The majority(69.51%) were peasants or migrant workers and almost all patients(96.51%) were Han people. Heterosexual transmission(96.02%) was the major route of HIV transmission among elderly patients. The proportions of heterosexual transmission among male and female patients were 95.30% and 98.00%, respectively. 68.96% of the patients were identified through preoperative examination, pre-transfusion examination and medical treatment examination.The results of the first CD4 cell test were less than 350 cells/m L in most the patients(67.56%).Conclusion HIV/AIDS epidemic spread rapidly among people aged 50 years or above and this population has become the key population of HIV infection. Targeted measures and comprehensive intervention strategies should be tailored to prevent HIV/AIDS among the population.
