伴CD20强阳性的小细胞变异型浆细胞骨髓瘤1例

<正>患者男性,63岁。腰部疼痛半年,加重1个月。腰椎磁共振示腰椎诸骨骨质信号普遍减低。实验室检查:血WBC8. 4×10~(12)/L,Hb 54 g/L,PLT41×10~9/L;尿本周氏蛋白阳性,Kappa游离轻链型;血清蛋白电泳发现M蛋白36. 65 g/L;血清免疫固定电泳示Ig G-κ型单克隆免疫球蛋白;血β2微球蛋白9. 6 mg/L,血肌酐浓度172μmol/L,血Ca 2. 46 mmol/L。行骨髓活检,送病理。病理检查巨检:骨髓环钻活检组织1条,大小0. 9 cm×
