24小时病历归档管理对DRGs实施的影响

摘要
目的研究24小时病历归档管理对诊断相关分组(DRGs)入组率及付费情况的影响。方法随机选取我院记录完整的病历作为研究资料,将病例实施24小时病历归档管理作为观察组,将病例均未实施24小时病历归档管理作为对照组,统计两组24和48小时病历归档率以及DRGs入组率和付费率,并对DRGs入组未付费的原因以及病案首页缺失情况进行分析。结果观察组24和48小时病历归档率以及DRGs入组率和付费率分别高于对照组,且观察组病案首页缺失率低于对照组,差异均有统计学意义(P<0.05)。在DRGs入组但未付费的原因中,观察组与对照组的主要原因都是"因病情需要出院,紧急结账"。结论 24小时病历归档管理可以提高DRGs入组率和DRGs付费率,降低病案首页缺失率,对DRGs的推行具有重要意义,利于有关机构对医疗费用的控制。
Objective To study the impact of 24-hour medical record archiving management on Diagnosis Related Groups(DRGs) enrollment rates and payment status. Methods The complete cases recorded in the hospital were randomly selected as the research objects. Cases administered 24 hours medical records archive management were regarded as observation group,and the cases who were not received 24 hours medical record filing management were regarded as a control group. The 24 and 48-hour medical record filing rates and the DRGs enrollment rate and payment rate were compared,and the reasons for the unpaid fees of the DRGs and the absence of the first page of the case were analyzed.Results The 24-hour and 48-hour medical record filing rate,and the DRGs enrollment rate and payment rate were higher in the observation group than those in the control group. The missing rate of the first page of the observation group was lower than that of the control group,and all the differences were statistically significant(P<0.05). Among the reasons why DRGs were enrolled but not paid,the main reason for the observation group and the control group was"discharge due to illness,and emergency checkout". Conclusion 24-hour medical record filing management can improve DRGs enrollment rate and DRGs payment rate,reduce the missing rate of the first page of the medical record,and is of great significance to the implementation of DRGs,which is conducive to the control of medical expenses by relevant institutions.
