针灸对运动应激大鼠免疫机能的调节作用

摘要
目的:
     观察针灸对长期剧烈运动应激状态下大鼠免疫功能的调节作用,并从神经-内分泌-免疫网络(HPA轴)的途径深入探讨针灸发挥作用的机制。
     方法:
     清洁级8周龄健康雄性SD大鼠59只,按数字随机分为四组:对照组(n=10),大强度游泳训练组(n=17),大强度游泳训练加艾灸组(n=16),大强度游泳训练加电针艾灸组(n=16)。以长期(8周)大强度(150min)游泳训练建立运动性免疫抑制大鼠模型,选用关元、百会、足三里三穴,采取单纯艾灸关元、百会、足三里三穴与电针双侧足三里穴加艾灸关元、百会穴的方式观察针灸干预对长期大强度运动应激状态下大鼠免疫机能的影响,并从神经—内分泌—免疫网络的角度深入探讨针灸干预作用的机制。运用放射免疫法测定大鼠下丘脑ACTH、CRH、CORT及IL-2的浓度;运用流式细胞仪技术测定大鼠T淋巴细胞亚群CD3~+、CD4~+、CD8~+、CD161~+(NK细胞)的数量;运用ELISA法测定大鼠血清SIL-2R、γ-IFN的浓度;运用~3H-TdR渗入法测试大鼠血淋巴细胞对有丝分裂原PHA的体外增殖能力。
     结果:
     与对照组比,大强度训练组的IL-2、γ-IFN显著性下降(P<0.01,P<0.05),NK细胞也有下降趋势(P>0.05),提示8周大强度游泳训练导致大鼠IL-2-IFN-NKC免疫调节网出现紊乱;尽管艾灸+大强度训练组的IL-2、γ-IFN、NK细胞均低于对照组,但IL-2没有统计学意义(P>0.05),同时与大强度训练组相比较,艾灸+大强度训练组的γ-IFN、IL-2在数值上仍高于大强度训练组;尽管电针+艾灸+大强度训练组IL-2、γ-IFN、NK细胞的也均出现下降,但与对照组比均无统计学意义(P>0.05),且其γ-IFN在数值上仍显著高于大强度训练组(P<0.05)。针灸各组间比较,艾灸+大强度训练组的γ-IFN、NK细胞低于电针+艾灸+大强度训练组,但无统计学意义(P>0.05)
     与对照组比,大强度训练组的大鼠体重显著性下降(P<0.01),SIL-2R却显著性升高(P<0.05),而艾灸+大强度训练组与电针+艾灸+大强度训练组的大鼠体重尽管也显著地低于对照组(P<0.01),但在数值上仍高于大强度训练组(P>0.05),同时,针灸二组的SIL-2R不仅均非常显著性低于大强度训练组(P<0.01),而且均显著性低于对照组(P<0.01)。
     与对照组比,大强度训练组的CD3~+、CD4~+/CD8~+比值、NK细胞均出现下降,其中CD3~+具有显著性意义(P<0.05),而其CD4~+尽管略有升高,但因其CD8~+也出现升高,故其CD4~+/CD8~+的比值仍有下降趋势(P>0.05);而艾灸+大强度训练组的CD3~+、CD4~+、CD4~+/CD8~+的比值均出现升高,其中CD4~+具有显著性意义(P<0.05),同时,CD3~+在数值上非常显著高于大强度训练组(P<0.01),但艾灸+大强度训练组的NK细胞出现具有统计学意义(P<0.05)的下降;电针+艾灸+大强度训练组的T淋巴细胞亚群各指标除NK细胞略有下降(P>0.05)外,其余均出现升高,其中CD3~+、CD4~+具有非常显著性意义(P<0.01,P<0.01),同时,电针+艾灸+大强度训练组的T淋巴细胞亚群各指标在数值上均高于大强度训练组,其中CD3~+、CD4~+具有显著性意义(P<0.01,P<0.05)。
     与对照组比,大强度训练组的脾脏指数、胸腺指数、淋巴细胞增殖能力均显著性下降(P<0.01,P<0.01,P<0.05),而艾灸+大强度训练组与电针+艾灸+大强度训练组的三项指标尽管也有下降趋势,但均无统计学意义(P>0.05),同时,在数值上,针灸二组的脾脏指数、胸腺指数仍非常显著高于大强度训练组(P<0.01)。
     与对照组相比,大强度训练组的CRH、ACTH、CORT均出现升高,其中ACTH、CORT具有显著性意义(P<0.01,P<0.05),而艾灸+大强度训练组的CRH、CORT有下降趋势,但无显著性意义(P>0.05),尽管艾灸+大强度训练组的ACTH显著高于对照组(P<0.01),但与大强度训练组相比较,其CRH、ACTH、CORT均出现降低,其中ACTH、CORT具有非常显著性意义(P<0.01);电针+艾灸+大强度训练组的CRH、ACTH、CORT与对照组相比较,CRH、ACTH略有升高,而CORT却有下降趋势,但均无显著性意义(P>0.05),而与大强度训练组相比较,电针+艾灸+大强度训练组的CRH、ACTH、CORT均出现降低,其中ACTH、CORT也具有非常显著性意义(P<0.01);针灸各组间比较,电针+艾灸+大强度训练组的CRH、CORT略高于艾灸+大强度训练组,但无显著性意义(P>0.05),而电针+艾灸+大强度训练组的ACTH显著低于艾灸+大强度训练组(P<0.05)。
     1、经过8周大强度游泳应激后,大鼠的神志、外观、体重等一般状况恶化,大鼠血液中的主要免疫效应指标IL-2、γ-IFN、NKC、T淋巴细胞亚群中的CD3~+、CD4~+、CD4~+/CD8~+的比值及大鼠脾脏指数、胸腺指数、血淋巴细胞对有丝分裂原PHA的体外增殖能力均出现显著性下降,而免疫抑制因子SIL-2R含量、HPA轴中CRH、ACTH、CORT均出现显著性升高。
     2、单纯艾灸百会、关元、足三里穴不仅可提高CD3~+的百分率,而且可显著提高辅助性T细胞CD4~+的百分率、下调抑制性T细胞CD8~+的浓度,引起CD4~+/CD8~+的比值升高,同时可以一定程度地阻止长期大强度运动应激状态下大鼠胸腺指数、脾脏指数及血淋巴细胞对有丝分裂原PHA的体外增殖反应的下降,但对长期大强度运动应激状态下NK细胞数量的下降缺乏正向调节作用。艾灸对长期大强度运动应激状态下IL-2-IFN-NKC免疫调节网的正向调节作用也比较有限,但仍可一定程度地阻止IL-2的下降。
     3、电针双侧足三里穴加艾灸百会、关元穴不仅可以有效下调NK细胞下降的幅度、升高CD4~+/CD8~+的比值,而且对CD3~+、CD4~+具有明显的促进作用,同时也可有效地阻止长期大强度运动应激状态下大鼠的胸腺指数、脾脏指数及血淋巴细胞对有丝分裂原PHA的体外增殖反应的下降。对长期大强度运动应激状态下IL-2-IFN-NKC免疫调节网的正向调节作用较单纯的艾灸更为全面,其可以有效地阻止大鼠IL-2、γ-IFN及NK细胞数量的下降。
     4、无论是单纯的艾灸还电针+艾灸的方式均可通过下调ACTH、CORT和CRH的含量,从3个层次对HPA轴的激素进行调节,改善HPA轴的功能,避免HPA轴功能持续过度亢进,从而实现对因长期剧烈运动应激引起的神经内分泌紊乱的良性干预作用。
     结论:
     1、长期大强度运动应激不仅可以导致大鼠的神经、外观、体重等一般情况恶化,同时可引起大鼠免疫器官结构萎缩、功能退化、免疫细胞数量减少、免疫分子分泌抑制,IL-2-IFN-NKC免疫调节网紊乱,表明大鼠出现运动性免疫抑制。而长期大强度运动应激引起HPA轴功能持续过度亢进,导致大鼠体内CORT过量合成,从而导致神经内分泌免疫功能紊乱,可能是导致机体免疫功能出现抑制(运动性免疫抑制)的主要原因。
     2、针灸可以减轻长期而剧烈的运动应激对机体免疫功能的损害,有效避免机体出现运动性免疫抑制,其作用的机制一方面可能是通过纠正HPA轴的功能紊乱,进而下调CORT的浓度来实现对机体在长期大强度运动应激状态下免疫功能的保护效应,有效地避免了运动性免疫抑制的发生;另一方面也可能与直接调整紊乱的IL-2-IFN-NKC免疫调节网进而良性调节免疫机能存在密切联系。
     3、针灸对长期大强度运动应激状态下大鼠紊乱的HPA轴功能的调节作用是多途径,多层次的,不同的针灸方式发挥良性干预作用的途径也可能存在差异,本次研究结果显示:针+灸的方式对大鼠免疫机能的调节作用较单纯的艾灸更为全面。
Objective:To study the influence that the Acupuncture pay on the mouse of long-term acute sport,and probe into the mechanism of Acupuncture nerve-internal secretion-immunity network(HPA).
     Methods:59 Clean 8-week-old healthy male SD rats,according to numbers,were randomly divided into four groups:control group(n=10),high-intensity swimming training group(n=17),the group of high-intensity swimming training with Moxibustion and Acupuncture(n=16),the group of high-intensity swimming training with moxibustion and electro-Acupuncture(n=16).Long-term(8 weeks) high-intensity (150min) swimming training exercise-induced immunosuppression to establish the rat model,select the three points of Guanyuan,Baihui,Zusanli,put simple moxibustionon three points of Guanyuan,Baihui,Zusanli,and put electro-Acupuncture on the point of Zusanli while put Moxibustion on the points of Guanyuan,Baihui,and then observe Acupuncture intervention on long-term high-intensity exercise stress state of the immune function of rats.From the way of nerve-endocrine-immune network,probe into the mechanism of Acupuncture intervention.Measured by radioimmunoassay using rat hypothalamic ACTH,CRH,CORT and IL-2 concentration;the use of flow cytometry determination of rat T-lymphocyte subsets CD3~+、CD4~+、CD8~+、CD161~+(NK cells) number; use ELISA determination of serum SIL-2R,γ-IFN concentration;the use of 3H-tdr infiltration method of testing blood lymphocyte mitogen in vitro proliferation of PHA.
     Results:
     Comparing to the control group,the IL-2,γ-IFN of the high-intensity training group decreased significantly((P<0.01,P<0.05),NK cells are on a downward trend(P>0.05), which imply that the eight-weeks high-intensity swimming training resulted in rats'IL-2, IFN-NKC immunoregulatory network disorder occur;Although the IL-2,γ-IFN,NK cells of the group of high-intensity training with Moxibustion are lower than that of the control group,but IL-2 is not statistically significant(P>0.05).Compared to the group of the high-intensity training,the values of theγ-IFN,IL-2 of the group of high-intensity training with moxibustionare still higher than that of high-intensity training group;Although the IL-2,γ-IFN,NK cells of the group of high-intensity training with electro-Acupuncture and moxibustion also decline,but it is not of statistical significance(P>0.05).Comparing to control group,itsγ-IFN in the value still significantly higher than that of the high-intensity training group(P<0.05).Comparing each other,theγ-IFN,NK cells of the group of high-intensity training with Acupuncture and moxibustion+ are less than that of the high-intensity training with electro-Acupuncture and moxibustion,but no statistical significance(P>0.05).
     Comparing to the control group,the weighs of the rats of the group of the high-intensity training decreased significantly(P<0.01),the SIL-2R is significantly higher(P<0.05), and the weighs of the rats of the group of the high-intensity training with electro-Acupuncture and Moxibustion are significantly lower than the control group (P<0.01),but still higher than that of high-intensity training group(P>0.05),the SIL-2R of the two groups with Acupuncture is not only significantly lower than that of the high-intensity training group(P<0.01),but were significantly lower than that of the control group(P<0.01)。
     Comparing to the control group,CD3~+、the ratio of CD4~+/CD8~+,and the NK cells of the high-intensity training group decrease significantly(P<0.05),while CD4~+ increase slightly,but CD8~+ also increased,so the ratio of CD4~+/CD8~+ still downward trend(P>0.05);CD3~+、CD4~+、the ratio of CD4~+/CD8~+ of the group of high-intensity training with Moxibustion increased in both,in which CD4~+ with significance(P<0.05),CD3~+ is very significantly higher than that of high-intensity training group(P<0.01),but NK cells of the group of high-intensity training with Moxibustion decrease significantly(P<0.05);The T-lymphocyte subsets of the indicators except the NK cells of the group of high-intensity training with lectro-Acupuncture and Moxibustion decreased slightly(P>0.05),the remaining increase in both,including CD3~+、CD4~+ with a very significant significance (P<0.01,P<0.01),the indexes of the T-lymphocyte subsets of the group of high-intensity training with electro-Acupuncture and Moxibustion are higher than that of high-intensity training group,CD3+,CD4+ are of significance(P<0.01,P<0.05).
     Comparing to the control group,the spleen index,thymus index,the proliferation ability of lymphocytes of the high-intensity training group were significantly reduced(P<0.01, P<0.01,P<0.05),and the three indexes of the group of the high-intensity training group with Moxibustion and electro-Acupuncture and the group of high-intensity training with Moxibustion are on a downward trend though,but no statistical significance(P>0.05),in the numerical,the spleen index and thymus index of the two groups with Acupuncture are significantly higher than that of high-intensity training group(P<0.01).
     Comparing to the control group,the CRH,ACTH,CORT of the high-intensity training group increased all,in which ACTH,CORT are significant(P<0.01,P<0.05),and the CRH,CORT of the group of high-intensity training with Moxibustion has been a downward trend,but no significant(P>0.05),despite the ACTH of the group of high-intensity training with Moxibustion significantly higher than that of the control group (P<0.01),but comparing to the group of high-intensity training the CRH,ACTH,CORT reduced all,in which ACTH,CORT is very significant(P<0.01) statistically;Compared to the control group,CRH,ACTH of the group of high-intensity training with electro-Acupuncture and Moxibustion slightly increased,and CORT is downward trend, but they are not significant(P>0.05),compared to the group of high-intensity training group,the CRH,ACTH and CORT of the group of high-intensity training with electro-Acupuncture and Moxibustion reduce all,in which ACTH,CORT are significant (P<0.01) statistically.Comparing each other amongst Acupuncture groups,the CRH, CORT of the group of high-intensity training with electro-Acupuncture and Moxibustion are slightly higher than that of the group of high-intensity training with moxibustion,but it is not significant statistically(P>0.05),the ACTH of the group of high-intensity training with Moxibustion and electro-Acupuncture are significantly lower than that of the group of high-intensity training with moxibustion(P<0.05).
     1.After high-intensity swimming stress of eight weeks,mind,appearance,body weight of rats are deteriorated,the rats' blood immune indexes:IL-2,γ-IFN,NKC,CD3~+、CD4~+、ratio of CD4~+/CD8~+ of T-lymphocyte subsets,and the spleen index,thymus index,blood lymphocytes to mitogen in vitro proliferation of PHA showed a significant decrease,while the immunosuppressive factor SIL-2R concentration,HPA axis in CRH,ACTH,CORT were significantly increased.
     2.Simply putting moxibustionon Baihui,Guanyuan,Zusanli not only can improve the percentage of CD3~+,and can significantly increase the percentage of CD4~+ of T helper cells,reduced concentration of CD8~+ of suppressive T cells,caused the ratio of CD4~+/CD8~+ increased,to some extent can prevent the decline of proliferative response of thymus index,spleen index and blood lymphocytes to PHA mitogen under the state of long-term high-intensity exercise stress,but decline of NK cells' quantity lack of positive regulatory role under the state of long-term high-intensity exercise stress.Moxibustionplay limited regulatory role on IL-2-IFN-NKC immunoregulatory network,it can still prevent to some extent the decline of IL-2.
     3.Electro-Acupuncture on the points of Bilateral Zusanli and Moxibustion on the points of Guanyuan and Baihui can not only effectively reduced the decrease in NK cells,increased the ratio of CD4~+/CD8~+,but also promote CD3~+、CD4~+ with a significant role,can also effectively prevent the decline vitro proliferative response of thymus index,spleen index and blood lymphocytes to PHA mitogen.o rats under the state of long-term high-intensity exercise stress.They can play a more comprehensive positive role on adjusting IL-2-IFN-NKC immunoregulatory network than simply Moxibustion under the state of long-term high-intensity exercise stress,which can effectively prevent decline in the number of IL-2,γ-IFN and NK cells of the rats.
     4 Whether simple Moxibustion or electro-Acupuncture combined with Moxibustioncan regulate the hormone of the HPA axis from the three levels,improve the function of HPA axis to avoid HPA axis function sustained over-hyper,through reducing ACTH,CORT and CRH content.It can achieve a benign intervention role disorders of the neuroendocrinef induced by long-term strenuous exercise stress.
     Conclusions:
     1 Long-term high-intensity exercise stress can not only lead to the nerve appearance, body weight of rats deteriorated,and can cause shrinkage of the structure of immune organs of rats,the functional degradation,reduction in the number of immune cells,inhibit secretion of immune molecules,IL-2-IFN-NKC immunoregulatory network disorder,show that exercise induced rats immunosuppression.And long-term high-intensity exercise stress cause the continued excessive HPA axis hyperfunction,resulting in excessive synthesis of CORT rats,and then resulting in neuroendocrine-immune dysfunction,which may be the main reasons of leading to emergence of immune function suppression(exercise-induced immune suppression)
     2 Acupuncture can reduce the long-term strenuous exercise stress on the immune function of the damage,and can effectively prevent exercise-induced immunosuppression,its mechanism may be:on the one hand,correcting dysfunction of the HPA axis,and then down the concentration of CORT to achieve the protective effect of immune function of the body of long-term high-intensity exercise stress state,and effectively prevent the movement of the occurrence of immunosuppression;on the other hand,related to directly adjustment disorder of IL-2-IFN-NKC immunoregulatory network and then positive regulation of closely linked immunodeficiency.
     3 the regulation that Acupuncture pay on HPA axis function disorder of rat of long-term high-intensity exercise stress of rats is multi-channel and multi-level,different methods of Acupuncture intervention play a positive role differently,the results of this study show: Acupuncture combined with Moxibustion pay more comprehensive adjustment on the immune function of rats than the simple Moxibustion.
