针刺后溪、申脉对青年健康人大脑中动脉影响的研究

摘要
目的:
     通过经颅多普勒超声仪连续观测针刺后溪、申脉穴在不同时间点对青年健康人大脑中动脉(MCA)血流动力学的变化情况,探讨针刺后溪、申脉穴对大脑中动脉(MCA)血流动力学的影响及与时间相关性。
     方法:
     随机选取10例青年健康志愿者(其中男5例,女5例,年龄18-25岁,平均22.00±1.62岁,均无吸烟、酗酒史,无晕针史),在安静的环境下,受试者取仰卧位,休息15分钟后,头偏向一侧,充分暴露检查一侧的颞部,使用经颅多普勒超声仪探头于颞窗探及大脑中动脉(MCA),针刺被测者的双侧后溪、申脉穴,在针刺前10分钟,针刺得气时,留针15分钟(行针平补平泻),留针30分钟(出针时),出针后10分钟五个时间点,在彩色最饱和处取样,检测大脑中动脉血流动力学的变化,记录收缩期最大血流速度(Vs)、舒张期最大血流速度(Vd)、平均血流速度(Vm)、搏动指数(PI)及阻抗指数(RI)并进行统计学分析。
     结果:
     针刺后溪、申脉穴,在针刺得气时,留针15分钟(行针平补平泻),留针30分钟(出针时),出针后10分钟Vd、Vs、RI、PI血流速度与针刺前10分钟相比较,均无统计学意义(P>0.05);针刺得气时与针刺前10分Vm相比,无统计学意义(P>0.05);留针15分钟(行针平补平泻)Vm血流速度与针刺前10分钟相比较有显著统计学意义(P<0.05);留针30分钟(出针时),出针后10分钟Vm血流速度与针刺前10分钟相比较有非常显著的统计学意义(P<0.01)。
     结论:
     1、针刺后溪、申脉穴在针刺过程中能显著增加大脑中动脉(MCA)的平均血流速度(Vm)。
     2、针刺后溪、申脉穴在针刺过程中可能增加大脑中动脉(MCA)的血流量。
     3、针刺后溪、申脉穴对大脑中动脉(MCA)的血管阻力无显著影响,提示针刺后溪、申脉穴对大脑中动脉无负性治疗作用。
     4、针刺后溪、申脉穴对大脑中动脉(MCA)的平均血流速度(Vm)有非常显著的后遗效应。
Objective:
     We observed the Trans Cranial Doppler Ultrasonography (TCD) of Middle Cerebral Artery (MCA) to study the therapeutic mechanism and the effect of the HouXi and ShenMai points with acupuncture.
     Methods:
     We evaluated the Trans Cranial Doppler Ultrasonography (TCD) of the Middle Cerebral Artery (MCA) before、during and after the period of treatment with acupuncture on the HouXi and ShenMai points of the tetraplegia and withdrawing the needle.
     We should aim for the peak velocity of systolic (Vs)、the peak velocity of diastolic (Vd)、the velocity of mean(Vm)、the pulsate index (PI) and the resistance index (RI) of the Middle Cerebral Artery (MCA).
     Results:
     The Trans Cranial Doppler Ultrasonography of the Middle Cerebral Artery is obvious change after the treated with acupuncture except the velocity of mean of the Middle Cerebral Artery on the five time points, so there are the significance of statistics.
     Conclusion:
     1 Acupuncture could raise the velocity of mean of the Middle Cerebral Artery when the HouXi and ShenMai points were treated.
     2 Acupuncture may raise the rate of flow of the Middle Cerebral Artery when the HouXi and ShenMai points were treated.
     3 Acupuncture could no raise the resistance of blood vessel of the Middle Cerebral Artery when the HouXi and ShenMai points were treated.
     4 Acupuncture could raise the marked after-effect of mean of the Middle Cerebral Artery when the HouXi and ShenMai points were treated.
