针灸“关元”穴干预营养性肥胖大鼠脂联素的实验研究

摘要
目的:
     本实验采用针灸疗法,经过观察治疗前后各组营养性肥胖大鼠体重和体重差值的变化,以及对血糖、胰岛素及脂联素水平的影响,以探讨针灸治疗单纯性肥胖的作用机制。
     方法:
     先将60只雄性SD大鼠随机分为两组:空白组10只和造模组50只。空白组以普通饲料喂养,造模组以高脂饲料喂养。同期喂养9周后,以造模组的体重超过空白组平均体重20%以上为营养性肥胖大鼠造模成功。将造模成功的24只随机分为模型组8只、艾灸组8只、针刺组8只。空白组和模型组仅束缚固定;艾灸组和针刺组取“关元”穴干预,每日1次,连续治疗20天,治疗期间各组大鼠均以普通饲料喂养,自由摄食和饮水。分别于治疗前、治疗第10天、治疗第20天后测量体重,实验结束后,将大鼠处死,迅速分离肾周脂肪和睾周脂肪分别称重,检测血清血糖、胰岛素、脂联素的水平。
     结果:
     1体重:造模组的体重高于空白组平均体重的20%以上为造模成功。治疗前模型组、艾灸组、针刺组之间体重比较差异无统计学差异(P>0.05)。治疗后模型组、艾灸组、针刺组的体重均比空白组高,有显著性差异(P<0.05),但艾灸组和针刺组较模型组明显降低,其差异亦有统计学意义(P<0.05),艾灸组和针刺组之间无统计学差异(P>0.05)。从治疗前后各组大鼠体重差值来看,艾灸组和针刺组明显低于空白组和模型组,而且具有显著性差异(P<0.05)。艾灸组和针刺组之间无统计学差异(P>0.05)。
     2脂肪湿重:模型组、艾灸组、针刺组的肾周和睾周脂肪湿重均明显高于空白组,且其差异有统计学意义(P<0.05)。针灸治疗后,艾灸组和针刺组的脂肪湿重较模型组明显减少,且具有显著性统计学差异(P<0.05)。艾灸组和针刺组之间的脂肪湿重无统计学差异(P>0.05)。
     3血糖:模型组、艾灸组、针刺组的血清血糖含量均较空白组高,并有显著性差异(P<0.05)。艾灸组和针刺组较模型组显著降低(P<0.05),与空白对照组无统计学意义(P>0.05)。艾灸组和针刺组之间无统计学差异(P>0.05)
     4胰岛素:模型组、艾灸组、针刺组的血清胰岛素含量和胰岛素抵抗指数均较空白组高,并有显著性差异(P<0.05)。艾灸组和针刺组较模型组显著降低(P<0.05),与空白对照组无统计学意义(P>0.05)。艾灸组和针刺组之间无统计学差异(P>0.05)
     5脂联素:模型组、艾灸组、针刺组的血清胰岛素水平明显低于空白组,并有显著性差异(P<0.05)。艾灸组和针刺组较模型组显著升高(P<0.05)。但艾灸组与针刺组之间无统计学差异(P>0.05)。
     结论:
     1.针灸可降低营养性肥胖大鼠的体重,起到减肥的作用。
     2.针灸可降低营养性肥胖大鼠的Glu、INS含量、IR指数,有效改善糖代谢紊乱及IR而达到减肥的目的。
     3.针灸可升高营养性肥胖大鼠APN水平,说明针灸对血清APN水平具有良性调节作用而达到减肥的效应
Objective:
     This experiment to study the underlying mechanism of acupuncture and moxibustion for simple obesity by investigating the influence the relevant indexes in nutritional obese rats, including the body weight, the weight differences between before and after treatment, the serum level of glucose, insulin and adiponectin.
     Methods:
     60male SD rats were randomly divided into two groups:10rats in the blank control group and50rats in the model group. During the period modeling, the blank control group rats were fed with normal diet, the model group rats were fed with high fat diet. After nine weeks, the rat weight of the model group is20%heavier than the average weight of the blank control group, it means the establishment of model was successed. There are24rats were made model success and randomly divided them into three groups:8rats in the model group,8rats in the moxibustion group,8rats in the acupuncture group. The treatment duration was20days, the rats of each groups were fed with normal diet, free feeding and drinking. Rats in blank control group and the model group were only fixed; Rats in moxibustion group and acupuncture group were received treatment at "Guan yuan" acupuncture point. The treatment had been conducted for15min daily for20days. During the modeling and treatment period, weighing the weight of the rats in each group, after the end of the experiment, fats aroud perirenal area and epididymides area were seperated and weigh there weight. And the level of blood serum Glucose,Insulin,Adiponectin factors were measured.
     Results:
     (1) The body weight:During the period of modeling, the weight of the model group is20%heavier than the average of blank control group, it means the establishment of model was successful. Before treatment, no significant difference among model group, moxibustion group, acupuncture group (P>0.05). After treatment,the weight of the model group, the moxibustion group, the acupuncture group considerably higher than the blank group, with a significant difference (P<0.05). Compared with the model group the moxibustion group and the acupuncture group significantly reduced, their difference was also statistically significant (P<0.05). No significant difference between the moxibustion group and the acupuncture group (P>0.05). From the weight differences on rats between before and after treatment, the moxibustion group and the acupuncture group was significantly lower than the blank control group and the model group, with significant differences (P <0.05).
     (2) Wet weight of fat:Compared with the blank control group, the wet weight of the area perirenal and epididymides area fat higher in the model group, the moxibustion group, the acupuncture group with significant difference (P<0.05). After treatment, the wet weight of the fat in moxibustion group and acupuncture group was decreased, compared with the model group, there was a significant statistical difference (P<0.05). No significant difference between the moxibustion group and the acupuncture group (P>0.05).
     (3) Glucose:Compared with the blank control group, the serum level of glucose significantly increased in the model group, the moxibustion group, the acupuncture group, and there was a significant difference (P<0.05). The glucose level in the moxibustion group and the acupuncture group were significantly lower than that of the model group (P<0.05), with the blank control group without statistical significance (P>0.05). No significant difference between the moxibustion group and the acupuncture group (P>0.05)
     (4) Insulin:Compared with the blank control group, the serum level of insulin and insulin resistance index significantly increased in the model group, the moxibustion group, the acupuncture group, with a significant difference (P<0.05). The serum level of insulin the moxibustion group and acupuncture group were significantly lower than that of the model group (P <0.05), with the blank control group without statistical significance (P>0.05). No significant difference between the moxibustion group and the acupuncture group (P>0.05)
     (5) Adiponectin:Compared with the blank control group, the serum level of adiponectin significantly decreased in the model group, the moxibustion group, the acupuncture group, and there was a significant difference (P <0.05).After the treatment, the serum adiponectin contents of the moxibustion group and the acupuncture group were considerably higher than that of the model group (P<0.05), with the blank control group without statistical significance (P>0.05). No significant difference between the moxibustion group and acupuncture group (P>0.05)
     Conclusion:
     1. Acupuncture and moxibustion can lower the body weight of nutritional abese rats, which is effective in reducing body.
     2. Acupuncture and moxibustion can decrease the serum level of glucose, insulin and the insulin resistance level of the nutritional abese rats, through that glycometabolism disorder in body is regulated, which can achieve reducing body.
     3. Acupuncture and moxibustion treatment can raise the serum level of adiponectin in nutritional obese rats, which may contribute to its effect in reducing body.
