外侧缰核介导岛叶皮层控制中缝核释放5-HT

摘要
目的边缘系统的岛叶皮层和缰核通过控制中缝核5-HT释放参与动物和人类呼吸困难的调节。由于缰核分内外侧两个部分,岛叶信号是通过内侧缰核、还是通过外侧缰核下行传导并不清楚。为了深入探讨在睡眠呼吸暂停综合征(Obstructive sleep apnea syndrome,OSA)发病中缰核及岛叶皮层的作用及功能关系,应用电刺激Wistar大鼠前岛叶皮层(anterior insular cortex,INS),记录呼吸曲线、内、外侧缰核神经元放电情况及动脉血气分析,并将记录结果与对照组(电刺激INS周围组)进行比较分析。结果电刺激INS大鼠出现呼吸紊乱及呼吸暂停;血p H值明显降低,剩余碱(BE)负值明显增大,与对照组比较具有显著差异(P<0.05);电刺激岛叶使大鼠内侧缰核神经元自发放电减弱,使外侧缰核神经元自发放电增加。结论电刺激岛叶皮层可诱发动物出现呼吸暂停,形成类似人类OSA的表现。岛叶皮层与缰核之间存在的功能联系,刺激岛叶皮层分别引起内、外侧缰核的抑制和兴奋。通过外侧缰核对中缝核的抑制加强,而导致OSA症状出现。从理论上阐述刺激岛叶皮层建立OSA动物模型的机制,也推进了对OSA中枢神经系统病因探讨。
