OBJECTIVE To explore the main factors associated with positional obstructive sleep apneahypopnea syndrome(OSAHS).METHODS 661 OSAHS patients were divided to non-positional obstructive sleep apnea(NPOSA) and positional obstructive sleep apnea(POSA).All patients were assessed by the Friedman staging system,Epworth Sleepiness Scale(ESS) questionnaire and anthropometry before overnight polysomonography.The blood test included fasting glucose and lipid profile tests.The forward conditional logistic regression analysis was used to assess the simultaneous contribution of all parameters on positional dependency and established prediction model for screening positional OSAHS patients.RESULTS There were 364 NPOSA patients and 297 POSA patients(55.1%vs 44.9%).Significant differences were found in the values of body mass index(BMI),neck circumference(NC),waist circumference(WC),hip circumference(HC),ESS score and AHI between NPOSA and POSA group,and POSA group had lower NC,WC,HC,BMI,ESS score,AHI,fasting glucose and apolipoprotein-B values than NPOSA group(p < 0.01).One model was PRE-1=0.066 × Min S02-0.026 × WC-2.436,which AUC was 0.741(95%CI:0.705 to 0.774),with sensitivity 71.7% and specificity 70.3%,and another model that only included AHI was PRE-2=-0.053×AHI +1.188,which AUC was 0.784(95%CI:0.751 to 0.815) with sensitivity 93.3%and specificity 54.9%.CONCLUSION The main correlative factors of POSA were AHI,Min SO2 and WC.The greater WC and more severity of OSAHS,the lower proportion of POSA was.
