摘要

目的:探讨桃红四物汤对老年膝关节置换患者血流动力学、心功能及凝血指标的影响,为临床治疗关节置换出现的心肺功能障碍并发症提供参考与思路。方法:选取60例老年膝关节置换患者,随机分为对照组与实验组,对照组30例患者采用常规治疗,实验组30例患者在常规治疗的基础上添加桃红四物汤口服治疗。治疗7天后观察两组患者的血流动力学、心功能指数与凝血指标的变化。结果:治疗前,两组患者血流动力学、心功能及凝血各项指标比较,无统计学意义(P>0.05)。治疗7天后,对照组患者较治疗前,心率(HR)、心输出量(CO)、心脏排血指数(CI)、每搏输出量(SV)与射血分数(EF)降低(P<0.05);中心静脉压(CVP)与肺动脉楔压(PAWP)增高(P<0.05);凝血酶原时间(PT)、活化部分凝血酶时间(APTT)与凝血酶时间(TT)降低(P<0.05),纤维蛋白原(FIB)指标增高(P<0.05)。实验组患者较治疗前,HR、CO、CI、SV与EF指数升高(P<0.05),CVP与PAWP指数降低;PT、APTT与TT时间增高(P<0.05),FIB指数降低(P<0.05)。实验组与对照组相比,患者HR、CO、CI、SV与EF指数升高(P<0.05),CVP与PAWP指数降低;PT、APTT与TT时间增高(P<0.05),FIB指数降低(P<0.05)。结论:服用桃红四物汤,有助于改善患者血流动力与心功能,可预防关节置换术后心肺功能障碍并发症的发生。
Objective:To explore the effect of Taohong Siwu Decoction on hemodynamics,cardiac function scale and coagulation indicator of geriatric knee replacement patients,and supply reference and thinking for clinical treatment of cardiorespiratory dysfuntion in joint replacement.Methods:The 60 geriatric knee replacement patients were divided into one control group and one experimental group.The 30 patients of control group were treated with conventional therapy,and30 patients of experimental group were treated orally with Taohong Siwu Decoction on the basis of conventional therapy.The hemodynamics,cardiac function scale and coagulation indicator of all patients were evaluated after 7 days of treatment.Result:Before the treatment,no significant differences were found between two groups in hemodynamics,cardiac function scale and coagulation indicator.After 7 days of treatment,as compared with prior treatment,the heart rate(HR),cardiac output(CO),cardiac index(CI),stroke volume(SV) and ejection fraction(EF) of control group were decreased(P <0.05).The central venous pressure(CVP) and pulmonary artery wedge pressure(PAWP) of control group were increased(P < 0.05).The prothrombin time(PT),activated partial thromboplastin time(APTT) and prothrombin time(TT) of control group were decreased(P < 0.05).The fibrinogen(FIB) index of control group was increased(P < 0.05).As for experimental group,as compared with prior treatment,the HR,CO,CI,SV and EF of experimental group were increased(P < 0.05),the CVP and PAWP of experimental group were decreased(P < 0.05).The PT,APTT and TT of experimental group were increased(P < 0.05).The FIB of experimental group were decreased(P < 0.05).As compared with control group,the HR,CO,CI,SV and EF of experimental group were increased(P < 0.05),the CVP and PAWP of experimental group were decreased(P < 0.05).The PT,APTT and TT of experimental group were increased(P < 0.05).The FIB of experimental group were decreased(P < 0.05).Conclusion Orally treated with Taohong Siwu Decoction,the hemodynamics,cardiac function of patients could be promoted,which could occurring of cardiorespiratory dysfuntion after joint replacement.
