健脾温化消翳汤联合贝复舒治疗单疱病毒性角膜炎临床疗效及对血清炎症因子的影响

摘要
目的:探讨健脾温化消翳汤联合贝复舒治疗单疱病毒性角膜炎临床疗效及对血清炎症因子的影响。方法:选择2015年10月—2016年12月于本院就诊的86例单疱病毒性角膜炎患者为研究对象,应用随机数表法分为对照组和观察组,各43例。对照组应用贝复舒进行局部治疗,观察组患者在对照组基础上联合应用健脾温化消翳汤口服治疗,两组持续治疗3周。评价两组临床疗效,并观察两组症状消失时间及治疗前后两组患者血清炎性因子改善情况,同时随访至治疗后1年,观察两组复发情况。结果:观察组患者的治疗总有效率95. 35%(41/43),高于对照组的81. 40%(35/43),差异有统计学意义(P <0. 05);治疗期间,观察组刺激症状消失、充血消退、角膜溃疡愈合等时间均低于对照组,差异有统计学意义(P <0. 05);治疗前,两组IL-18、IFN-γ水平对比,差异无统计学意义(P> 0. 05);治疗后,两组IL-18、IFN-γ水平均高于治疗前,观察组高于对照组,差异有统计学意义(P <0. 05);随访1年内,观察组疾病复发率2. 33%(1/43),低于对照组的18. 80%(8/43),差异有统计学意义(P <0. 05)。结论:应用健脾温化消翳汤联合贝复舒治疗单疱病毒性角膜炎疗效显著,可有效改善患者血清炎症因子水平,提高细胞免疫功能,加速临床症状缓解,降低复发几率,预后良好。
Objective: To explore the clinical efficacy of Jianpi Wenhua Xiaoyi Decoction combined with b FGF Eye Drops in the treatment of herpes simplex virus keratitis and its effect on serum inflammatory factors. Methods: Eighty-six cases of patients with herpes simplex virus keratitis treated in our hospital from October 2015 to December 2016 were selected as the research subjects. All the patients were divided into control group and observation group according to random number table method,43 cases in each group. The patients in the control group were given b FGF Eye Drops for local treatment,and the patients in the observation group were treated with Jianpi Wenhua Xiaoyi Decoction on the basis of the control group,and both groups werecontinuously treated for 3 weeks. We evaluated the clinical efficacy of the two groups and observed the disappearance of symptoms and the improvement of serum inflammatory factors in both groups before and after treatment. At the same time,follow-up to 1 year after treatment,we observed the recurrence of the two groups. Results: The total effective rate of observation group was 95. 35%( 41/43),which was significantly higher than that of control group( 81. 40%,35/43)( P < 0. 05). During the course of treatment,the irritation symptom of observation group disappeared,the congestive subsidence and corneal ulcer healing time were lower than those of the control group and the difference was statistically significant( P < 0. 05). Before treatment,there was no significant difference between the two groups in the levels of IL-18 and IFN-γ( P > 0. 05). After treatment,the levels of IL-18 and IFN-γ in both groups were significantly higher than those before treatment,and the difference was statistically significant( P < 0. 05). After the follow-up of 1 year,the observation group' s disease recurrence rate was 2. 33%( 1/43),which was lower than the control group's 18. 80%( 8/43) and the difference was statistically significant( P < 0. 05). Conclusion: Jianpi Wenhua Xiaoyi Decoction combined with b FGF Eye Drops for the treatment of herpes simplex virus keratitis has significant effect,and can effectively improve the levels of serum inflammatory cytokines,improve cellular immune function,accelerate the clinical symptoms and reduce the chance of recurrence and the prognosis is good.
