个别托盘在Kennedy Ⅰ类患者中的研究应用

摘要
目的探讨研究选择个别托盘在Kennedy Ⅰ类牙列缺损患者中的应用效果。方法选择在河北北方学院附属第一医院口腔科2014年10月~2016年9月就诊的65例Kennedy Ⅰ类牙列缺损患者展开研究。按随机数字表法将患者分为对照组(32例)和观察组(33例)。对照组采用普通托盘制作印模,观察组采用个别托盘制作印模。患者佩戴义齿1周后复检,评价患者戴用义齿前后的咀嚼效能和义齿的固位力;1个月后随访患者戴用义齿的满意度。结果两组患者戴用义齿前咀嚼效能比较,差异无统计学意义(P>0.05);戴用义齿后两组咀嚼效能均明显提高,且观察组咀嚼效能高于对照组(P<0.05)。观察组固位力明显高于对照组(P<0.05)。观察组在语音和舒适度方面的满意度评分均明显高于对照组(P<0.05);但在美观方面两组的满意度评分方面差异无统计学意义(P>0.05)。结论利用个别托盘对Kennedy Ⅰ类牙列缺损患者进行印模制取,可以加强可摘局部义齿的咀嚼效能和固位力,语音能力恢复良好,舒适度得到保证,值得临床广泛应用。
Objective To Investigate the clinical curative application and effect of the individual impression tray in patients with Kennedy Ⅰ denture. Methods 65 patients with Kennedy Ⅰ from October 2014 to September 2016 in Department of Stomatology, the First Affiliated Hospital of Hebei North University were collected. They were divided into observation group(33 cases) and control group(32 cases) according to random number table. The patients in observation group were restored with individual impression tray and those in the control group were restored with common tray. One week after patients used denture, the masticatory efficiency and the retention force were evaluated. And one month after patients used denture, the satisfaction scores were evaluated. Results Before patients used denture, there was no difference in the masticatory between the two groups(P > 0.05). One week after patients used denture, the masticatory efficiency of the two groups was increased, and the masticatory of the observation group was higher than that of the control group(P < 0.05). The retention force of the observation group was higher than that of the control group(P < 0.05). One month after patients used denture, the satisfaction scores of phonological competence and comfort level of the observation group were better than those of the control group(P < 0.05), while there was no significantly difference in the aesthetics between the two groups(P > 0.05). Conclusion The individual impression tray of Kennedy Ⅰ denture can improve the masticatory efficiency, retention force, phonological competence and comfort level. It is worthy of clinical application
