老年髋关节骨折患者行手术治疗时不同麻醉方式对患者预后的影响

摘要
目的探讨老年髋关节骨折患者行手术治疗时不同麻醉方式对患者预后的影响。方法选择我院2017年1月至2018年3月收治的40例老年髋关节骨折患者作为研究对象;根据手术麻醉方式不同将患者分为对照组(20例,全身麻醉)和观察组(20例,外周神经阻滞麻醉)。比较两组患者ICU住院时间、DBP、SBP、HR的最大变化值。结果观察组患者ICU住院时间显著短于对照组,差异有统计学意义(P<0.05)。观察组DBP最大变化值、SBP最大变化值、HR最大变化值均小于对照组,差异有统计学意义(P<0.05)。结论在老年髋关节骨折患者手术治疗时采用外周神经阻滞的方法进行麻醉,可以缩短患者ICU住院时间、降低DBP、SBP、HR的最大变化值,从而保证老年髋关节骨折患者手术顺利实施,改善患者预后,加快病情康复,值得临床推广应用。
Objective To explore the effect of different anesthesia methods on the prognosis of elderly patients with hipfracture underwent surgical treatment. Methods A total of 40 elderly patients with hip fracture admitted to our hospital fromJanuary 2017 to March 2018 were selected as study objects, they were divided into control group(20 cases, generalanesthesia) and observation group(20 cases, peripheral nerve block anesthesia) according to different surgical anesthesiamethods. The ICU hospitalization time, the maximum changes in DBP, SBP and HR were compared between the two groups.Results The ICU hospitalization time in the observation group was significantly shorter than that in the control group, andthe difference was statistically significant(P<0.05). The maximum changes in DBP, SBP and HR in the observation groupwere smaller than those in the control group, and the differences were statistically significant(P<0.05). Conclusion Usingperipheral nerve block method to anesthetize in the surgical treatment of elderly patients with hip fracture can shorten theICU hospitalization time and decrease the maximum change of DBP, SBP and HR, thus ensuring the smooth implementationand improvement of the elderly patients with hip fracture, improve patient prognosis and speed up the recovery of thedisease, it is worthy of clinical application.
