摘要
目的了解临床医学研究生的工作倦怠情况及医患沟通能力,探讨其影响因素及两者的相关性。方法采用横断面调查方法,对569名临床医学研究生进行自填式问卷调查,收集其工作倦怠及医患沟通能力的相关资料。结果 569名调查对象中,431人(75.7%)存在工作倦怠,其中,轻、中和重度工作倦怠的分别有219人(38.5%)、160人(28.1%)、52人(9.1%)。不同人口学特征的临床医学研究生的工作倦怠情况及医患沟通能力不同,高年级可能是工作倦怠的保护因素(OR=0.370,95%CI 0.235~0.582),工作时间长和医患沟通能力弱可能是工作倦怠的危险因素(OR=2.923,95%CI 1.397~6.116;OR=1.981,95%CI 1.044~3.759)。结论工作倦怠与医患沟通能力存在一定的相关性,临床医学研究生的工作倦怠现状及医患沟通能力的培养应得到医学教育部门的高度重视。
Objective To investigate the job burnout situation and doctor-patient communication skill among clinical medical postgraduates in a medical college in Guangzhou, and to explore their influence factors. Methods A total of 569 clinical medical postgraduates were randomly investigated with self-administered questionnaire on a cross-sectional study. Results Jon burnout was revealed in 431(75.7%) clinical medical postgraduates, among whom 219(38.5%) had mild job burnout, 160(28.1%) had medium job burnout and 52(9.1%) had severe job burnout. The clinical medical postgraduates with different demographic characteristics had different job burnout situation and doctor-patient communication skill. Senior grade was the protective factor of job burnout(OR=0.370, 95%CI 0.235-0.582), and long working hours and the lack of doctor-patient communication skill were the risk factors of job burnout(OR=2.923, 95%CI 1.397-6.116; OR=1.981, 95%CI 1.044~3.759). Conclusion There is a correlation between job burnout and doctor-patient communication skill. The current situation of job burnout and the cultivation of doctor-patient communication skill in clinical medical postgraduates should be highly valued by medical education departments.
