上海某社区40岁以上人群糖代谢状态调查

摘要
目的 :分析上海市宝山区淞南镇40岁以上人群糖代谢状态,为社区糖尿病防控提供依据。方法 :采用分层随机抽样的方法,抽取淞南镇3个居委40岁以上常住居民1 182人,其中男性514人(43.49%),平均年龄(64.2±6.8)岁,女性668人(56.51%),平均年龄(63.0±8.5)岁。对调查对象实施问卷调查、体格检查、口服葡萄糖耐量试验(OGTT)。结果:糖尿病前期的检出率为22.08%;糖尿病的检出率为22.50%。在糖尿病患者中,有92例为新发现糖尿病,占34.59%。糖尿病前期检出率和糖尿病检出率男性均高于女性(P <0.05);男性糖尿病前期的检出率和女性糖尿病的检出率随年龄增长呈上升趋势(P <0.05)。多因素Logistic回归分析显示,三酰甘油升高、静态生活方式、超重或肥胖、血压异常是糖代谢异常的危险因素,高密度脂蛋白胆固醇为糖尿病的保护因素。结论 :上海淞南镇40岁以上居民的糖代谢异常患病率较高,开展针对性的社区健康教育和健康促进是今后防控2型糖尿病的主要方向。
Objective: To analyze the status of glucose metabolism of people over 40 years old in Songnan Community,Baoshan District, Shanghai, and to provide evidence for community diabetes prevention and control. Methods: A stratified random sampling method was used to extract 1 182 permanent residents over 40 years of age in 3 residents committees of Songnan Community, including 514 males(43.49%), with an average age of(64.2±6.8) years old and 668 females(56.51%),with an average age(63.0±8.5) years old. A questionnaire survey, physical examination, and oral glucose tolerance test(OGTT)were performed in survey respondents. Results: The pre-diabetes detection rate was 22.08%; the detection rate of diabetes was22.50%. Among the diabetic patients, 92 were newly diagnosed with diabetes, accounting for 34.59%. The pre-diabetes detection rate and diabetes detection rate were higher in males than in females(P<0.05); the detection rates of pre-diabetes in males and diabetes in females increased with age(P<0.05). Multivariate logistic regression analysis showed that elevated triglyceride, static lifestyle, overweight or obesity and abnormal blood pressure were risk factors for abnormal glucose metabolism, and HDL-c was a protective factor for diabetes. Conclusion: The prevalence of abnormal glucose metabolism is higher in residents over 40 years old in Songnan Community, Shanghai, and developing targeted community health education and health promotion is the main direction of preventing and controlling type 2 diabetes in the future.
