摘要
目的探讨血清脂肪细胞型脂肪酸结合蛋白4(FABP4)与胰岛素抵抗的关系。方法收集2016年1—12月十堰市太和医院收治的60例2型糖尿病(T2DM组)病人及60例健康体检者(对照组)的临床资料,并应用ELISA法测定两组血清FABP4水平,同时测定受试者各组空腹血糖水平(FBG)、血压、空腹胰岛素(FINS)、C肽等指标,并通过稳态模型评估胰岛素β细胞功能指数(HOMA-β)及胰岛素抵抗指数(HOMA-IR)分析产生胰岛素抵抗的独立影响因素。结果 T2DM病人体质量指数(BMI)、腰臀比显著高于对照组(P<0.05),T2DM病人血清FABP4、FBG、FINS、C肽、HOMA-IR水平高于对照组(P<0.05),HOMA-β水平低于对照组(P<0.05),所有受试者中共有67例为胰岛素抵抗,53例为非胰岛素抵抗,FBG高于6 mmol/L,C肽高于1.5 ng/mL,FABP4高于30 ng/L是导致受试者出现胰岛素抵抗的独立危险因素(P<0.05),以HOMA-IR是否大于等于2.80为"金标准",利用FBG,C肽和FABP4三个指标对120例受试者进行有无胰岛素抵抗的诊断,其诊断的正确指数分别为0.343,0.310,0.221。结论血清FABP4水平升高与胰岛素抵抗有关。
        Objective To investigate the resistance relationship between serum adipocyte fatty acid binding protein 4(FABP4)and type 2 diabetes mellitus(T2 DM).Methods The clinical data of 60 T2 DM patients and 60 healthy subjects in Shiyan City Taihe Hospital from Jannary 2016 to December 2016 were collected.The levels of serum FABP4 of two groups were measured by ELISA. The fasting blood glucose(FBG), blood pressure, fasting insulin(FINS) and Insulin Resistance Index(HOMA-IR) were measured by steady-state model.The relationship between FABP4 and insulin resistance in T2 DM patients was analyzed by person single factor and Logistic multivariate analysis. Results Body mass index in patients with T2 DM(BMI), waist to hip ratio were significantly higher than the control group(P < 0.05).T2 DM FBC, serum FABP4, FINS, C, HOMA-IR peptide levels were higher than the control group(P<0.05), and HOMA-levels lower than the control group(P< 0.05).In all subjects, 67 were insulin resistance while.53 were for non insulin resistance.Resistance to FBG is higher than 6 mmol/L,C peptide was higher than 1.5 ng/mL,FABP4 higher than 30 ng/L which is an independent risk factor for insulin resistance in patients with HOMA-IR(P<0.05), is greater than or equal to 2.80 as a "gold standard" using FBG,C peptide and FABP4 three indicators have no diagnosis, insulin resistance in 120 subjects the results showed that three indexes of diagnosis index were 0.343,0.310,0.221.Conclusion Elevated levels of serum FABP4 are associated with insulin resistance.
