当归四逆汤治疗老年退行性腰腿痛临床观察

摘要

目的探讨当归四逆汤治疗老年退行性腰腿痛的临床效果。方法选取2016年5月—2018年9月收治的68例退行性腰腿痛老年患者为研究对象,随机分为观察组和对照组,各34例。对照组采取西医常规治疗,观察组在对照组的基础上采用当归四逆汤治疗,2组均连续治疗1个月。比较2组患者治疗前后临床症状缓解情况及炎性因子水平。结果治疗后2组患者ODI评分较治疗前降低,JOA评分较治疗前升高(P <0. 01),且观察组较对照组改善程度更为显著(P <0. 01)。治疗后2组患者腰腿疼痛、腰背酸软、乏力疲劳、肢体麻木、少气懒言、口干咽燥等症状积分均较治疗前显著下降(P <0. 01),且观察组各积分较对照组下降更多(P <0. 01)。治疗后2组患者血清中IL-6、TNF-α、hs-CRP水平较治疗前显著降低(P <0. 01),且观察组较对照组下降更多(P <0. 01)。结论当归四逆汤治疗老年退行性腰腿痛可有效抑制炎症反应,缓解其疼痛及其他临床症状,改善腰部功能,效果显著优于西医常规治疗。
        Objective To investigate the clinical effect of Danggui Sini decoction in the treatment of senile degenerative lumbocrural pain.Methods 68 patients with senile degenerative lumbocrural pain admitted to our hospital from May 2016 to September 2018 were selected as the research objects,and they were divided into observation group and control group according to random number tables,with 34 cases in each group. The control group was treated with conventional western medicine,and the observation group was treated with Danggui Sini decoction on the basis of the control group. 2 groups were treated continuously for 1 month. The clinical symptom remission and inflammatory factor levels of 2 groups before and after treatment were compared. Results After treatment,ODI scores of the two groups were lower and JOA scores were higher than those before treatment( P < 0. 01),and the improvement of the observation group was more significant than that of the control group( P < 0. 01). After treatment,the scores of symptoms such as lumbocrural pain,lumbar and back acerbity and softness,fatigue and fatigue,numbness of limbs,lack of breath and lazy speech,dry mouth and pharynx in 2 groups significantly decreased compared with those before treatment( P < 0. 01),and observation group were significantly lower than control group( P < 0. 01). After treatment,serum IL-6,TNF-α,hs-CRP levels in 2 groups significantly decreased compared with those before treatment( P < 0. 01),and observation group were significantly lower than control group( P < 0. 01). Conclusion Danggui Sini decoction in the treatment of senile degenerative lumbocrural pain can effectively inhibit inflammation,relieve pain and other clinical symptoms,improve lumbar function,and the effect is significantly better than conventional western medicine treatment.
