剂量效应对抗体鉴定的影响

摘要
目的探讨剂量效应对抗体鉴定结果判定的影响。方法选取2018年北京市红十字血液中心微柱凝胶法(microcolumn gel test, MGT)明确为MNS、Rh、Kidd血型系统不规则抗体阳性的受血者样本30份,将受血者血清进行倍比稀释,分别选择谱细胞试剂中纯合子和杂合子细胞与不同浓度倍比稀释后血清反应,选取出现阴性结果的最高稀释度来评价与纯合子和杂合子抗原阳性谱细胞反应的凝集强度,以此观察剂量效应现象对不规则抗体鉴定的影响。结果经过稀释的30份抗体鉴定为阳性的血清样本中,经MGT法检测后,25份不规则抗体与纯合子谱细胞凝集强度≤2+,其中19份与杂合子谱细胞反应≤w+,6份杂合子与纯合子谱细胞反应强度相同;5份与纯合子谱细胞反应强度为3+,与杂合子谱细胞反应强度≥2+。结论 MNS、Rh、Kidd血型系统不规则抗体与纯合子抗原谱细胞反应强度≤2+时,杂合子抗原反应会减弱,出现剂量效应,进而影响抗体鉴定结果判定。
Objective To investigate the influence of dosage effect on irregular antibody identification. Methods A total of 30 blood samples containing definite irregular antibodies which was selected by microcolumn gel test(MGT) was diluted to obtain different concentrate liquid. The concentrated was reacted with single dosage and double dosage on panel cells,then the highest concentration with a negative result was choosed to evaluate the difference agglutination strength between double dosage and single dosage panel cells, and the dosage effect was observed on irregular antibody identification. Results Among the 30 irregular antibody positive samples detected by MGT, the agglutination strength of 25 samples reacted with double dosage was ≤2+, and 19 of them with single dosage was ≤W+. The agglutination strengths of six samples were same between double dosage and single dosage. The agglutination strengths of five samples reacted with double dosage were 3+,with single dosage were≥2+. Conclusions Serum reacted with single dosage maybe weaken when reaction with double dosage is≤2+ in MNS,Rh,Kidd blood system, and which will further influence the irregular antibody identification.
