江门地区健康人群胃蛋白酶原参考值建立的初步研究

摘要
目的检测健康人群PGⅠ、PGⅡ,建立江门地区健康人群胃蛋白酶原参考值,为胃恶性肿瘤、十二指肠溃疡等胃黏膜病变的诊断与鉴别提供科学的参考依据。方法选择2018年1—2018年6月3 927名在本院体检中心进行体检人群,采用乳胶免疫比浊法测定PGⅠ和PGⅡ的浓度并计算PGⅠ/PGⅡ比值,制定建立江门地区健康人群胃蛋白酶原参考值范围。结果 3 927名健康居民血清PG检测结果经Kolmogorov-Smirnov检验提示:江门地区的健康人群血清PGⅠ、PGⅡ和PGⅠ/PGⅡ均呈偏态分布(P<0.05),男性血清PGⅠ浓度和PGⅠ/PGⅡ比值显著上升(P<0.05),而不同血清PGⅡ浓度无显著性差异(P>0.05),不同年龄人群血清PGⅠ浓度无显著差异(P> 0.05);随着年龄的增加,血清PGⅡ浓度显著性升高,PGⅠ/PGⅡ比值随着年龄增加显著性下降(P<0.05),实验组的PG浓度数据均为偏态分布,所以采用百分位数法计算确定,由于研究表明不同性别居民的血清PGⅠ浓度存在显著性差异(P<0.05),因此要分别确定男性、女性血清PG的95%置信区间,PGⅠ、PGⅡ取双侧参考区间(P2.5-P97.5),PGⅠ/PGⅡ比值取单侧下限(≥P5)。结论确定了江门地区健康人群胃蛋白酶原参考范围,可为胃癌、十二指肠溃疡等疾病的预防、诊断治疗提供一定的参考依据。
        Objective To detect PGI and PGII in healthy people and establish reference value of pepsinogen in healthy population in Jiangmen area,and provide scientific reference for diagnosis and identification of gastric mucosal lesions such as gastric malignant tumor and duodenal ulcer. Methods 3927 people underwent physical examination in the physical examination center of our hospital from January 2018 to June 2018 were selected. The concentration of PGI and PGII was determined by latex immunoturbidimetry and the ratio of PGI/PGII was calculated. The reference range of pepsinogen in healthy population in Jiangmen area was established.Results The results of serum PG test in 3927 healthy residents were confirmed by Kolmogorov-Smirnov test:serum PGI,PGII and PGI/PGII in healthy population in Jiangmen area were skewed( P<0.05). The serum PGI concentration and PGI/PGII ratio of males increased significantly( P<0.05),there was no significant difference in serum PGII concentration( P> 0.05). There was no significant difference in serum PGI concentration among different age groups( P> 0. 05). Serum PGII concentration increased significantly with the increase of age. The PGI/PGII ratio decreased significantly with the inrease of age( P < 0. 05). The PG concentration data of the experimental group were all skewed,so the percentile method was used to determine the serum PGI concentration of different genders. Because the concentration of PGI between different genders with significant difference,so the95% confidence interval of serum PG in male and female should be determined separately. PGI and PGII should be in the bilateral reference interval( P2.5-P97.5),and the PGI/PGII ratio should select unilateral Lower limit( ≥P5). Conclusions The reference range of serum PG I and PG II and their ratios in healthy people could provide evidence for laboratory and clinical work. However,we only analyze the influence of sex and age on PG,and have some limitations. In the future,we need to further study the other influencing factors of serum PG in order to provide more help for diagnosis and treatment and prevention of gastric cancer,duodenal ulcer and other diseases.
