摘要
目的:研究双能量CT在急性缺血性脑卒中患者血管内治疗后颅内出血评估中的诊断价值,为患者的临床诊断提供指导。方法:选取2017年1月~2018年9月接诊的80例缺血性脑卒中患者为研究对象。在所有患者进行介入治疗术完成后2 h内进行头颅双能量CT平扫,进行图像处理后得到碘叠加图像(IOM)与虚拟平扫图像(VNC),选择140 kV与80 kV双能量模式进行扫描处理,以48 h后头颅CT复查所得的图像结合临床资料为金标准,判断双能量CT在急性缺血性脑卒中患者血管内治疗后颅内出血评估中的诊断价值。结果:计算结果为双能量CT诊断颅内出血有100%的敏感度,95.00%的特异性,87.50%的阳性预测率,100%的阴性预测率。结论:双能CT能够有效鉴别急性缺血性脑卒中血管内治疗术后碘对比剂外渗与早期脑出血,可为临床后续治疗提供依据。
Objective To study the diagnostic value of dual-energy CT in evaluating intracranial hemorrhage after intravascular treatment in patients with acute ischemic stroke.Method Eighty patients with ischemic stroke admitted to our hospital from January 2017 to September 2018 were selected for the study.All patients were scanned with dual-energy CT within 2 hours after interventional therapy.After image processing,iodine-overlapped image and virtual image were obtained.140 kV and 80 kV dual-energy mode were selected for scanning.48 hours later,combined with clinical data,dual-energy CT images were used as gold standard to judge the acute state of dual-energy CT.Diagnostic value of intracranial hemorrhage after intravascular treatment in patients with ischemic stroke.Results The results showed that dual-energy CT had 100% sensitivity,95.00% specificity,87.50% positive predictive rate and 100% negative predictive rate in the diagnosis of intracranial hemorrhage.Conclusion Dual-energy CT can effectively differentiate iodine contrast agent extravasation and early cerebral hemorrhage after endovascular treatment of acute ischemic stroke,and can guide the follow-up clinical treatment.
