路志正教授从“先天不足”论痹病的发病机制

摘要
痹病在临床中发病率高,破坏人体筋骨、肌肉、血管等多系统多脏腑功能,为历代中医所重视。《黄帝内经》认为痹病是由"风寒湿三气杂至合而为痹",后世医家对痹病发病机制亦多有论述。路志正教授为首届国医大师、中医临床大家,倡脾胃及温病学说,数十年研究中医痹病学,将历代中医理论与临床密切结合,以传统理论指导临床,以临床进一步发展理论。路老认为痹病的病因病机复杂,是内外因共同作用的结果,而内因在痹病发生中往往起到决定性因素。正气不足是痹病发生的内在原因基础,包括肾虚证及体质、遗传因素等造成先天不足是痹病发病重要因素之一。文章从先天不足角度结合理论、临床验案等简单阐述路志正教授对痹病发病机制的研究。
Bi disease is high in morbidity and damages many systems and zangfu functions with tendons,bones,muscles and vessels involved in human body. It has been greatly considered by TCM masters in dynasties. Huangdi Neijing( 《黄帝内经》) believes that Bi disease is caused by the interaction of pathogenic wind,cold and damp. The The medical masters in later dynasties had many discussions on the pathogenic mechanism of Bi disease. Prof. Lu Zhizheng,the national medicine master and TCM master in China. He advocates the theory of spleen and stomach and fibril disease. He has engaged in the study of Bi disease for several decades,integrated the TCM theory of dynasties with clinical practice and mutually developed and supplemented the traditional theories and clinical practice. Prof. Lu believes that the etiology and pathogenesis of Bi disease are complicated,the disease results from the interaction of internal factors and external factors. The internal factors play the key in the occurrence of Bi disease. Insufficiency of antipathogenic qi is the internal factor of disease. Kidney deficiency and the congenital deficiency due to body constitutional factor and inherent factor are important in the occurrence of disease. In view of the congenital deficiency,in coordination with the theory and clinical experiences,the paper explained briefly Prof. Lu Zhizheng' s study on the pathogenic mechanism of Bi disease.
