低温等离子射频消融术治疗小儿鼾症42例疗效探讨

摘要
目的研究探讨低温等离子射频消融术治疗小儿鼾症的临床疗效的观察。方法方便选择2016年1月—2018年12月在该院进行治疗的小儿鼾症患者42例为研究对象,按患儿住院先后顺序分为观察组和对照组,每组21例。对照组给予常规的电凝刀切除扁桃体腺样体,观察组患儿给予低温等离子射频消融术治疗。对两组患者的手术相关指标、应激功能指标、呼吸通气功能指标等进行统计对比。结果观察组患儿手术时间、术中出血量、术后疼痛(VAS)评分分别为(22.18±7.72)min、(18.12±5.50)mL、(1.38±0.43)分,均低于对照组患者为(31.32±8.54)min、(22.49±5.98)mL、(1.82±0.61)分,组间比较差异有统计学意义(t=3.638、2.465、2.702,P=0.001、0.018、0.010)。观察组患儿的谷胱甘肽(GSH)、超氧化物歧化酶(SOD)水平值分别为(71.29±12.39) U/mL、(74.57±13.56) U/mL高于对照组患者为(49.98±9.23) U/mL、(54.69±10.72) U/mL,观察组丙二醛(MDA)水平(10.38±3.23)mmol/L低于对照组患者为(15.23±4.20)mmol/L,组间差异有统计学意义(t=6.321、5.270、4.195,P=0.000、0.000、0.000)。观察组呼吸紊乱指数(AHI)、最长呼吸暂停时间(LAT)为(15.23±3.12)次/min、(17.82±3.98)s均低于对照组(18.97±3.42)次/min、(22.16±4.03)s,组间差异有统计学意义(t=3.702、3.511,P=0.001、0.001)。结论低温等离子射频消融术治疗小儿鼾症的效果显著,能够改善患儿通气功能,降低应激反应,值得推广应用。
        Objective To investigate the clinical effect of low temperature plasma radiofrequency ablation in the treatment of infantile snoring. Methods A total of 42 children with snoring who were treated in our hospital from January 2016 to December 2018 were convenient divided into observation group and control group according to the order of hospitalization, 21 cases in each group. The control group was treated with conventional electrocoagulation knife for tonsillectomy, and the observation group was treated with low temperature plasma radiofrequency ablation. The operation related indexes, stress function indexes and respiratory and ventilation function indexes were compared between the two groups. Results The operation time, intraoperative bleeding volume and postoperative pain(VAS) scores in the observation group were(22.18±7.72) min,(18.12±5.50) ml and(1.38±0.43) respectively, which were lower than those in the control group(31.32±8.54) min,(22.49 ±5.98) ml and(1.82±0.61). There were statistically significant differences between the two groups(t=3.638, 2.465, 2.702, P=0.001, 0.018, 0.010). The levels of glutathione(GSH) and superoxide dismutase(SOD) in the observation group were(71.29±12.39) U/mL,(74.57±13.56) U/mL, which were higher than those in the control group(49.98 ±9.23) U/mL,(54.69±10.72) U/mL. MDA level in the observation group was(10.38±3.23)mmol/L, which was lower than the control group(15.23±4.20)mmol/L. The respiratory disorder index(AHI) and the longest apnea time(LAT) of the observation group were(15.23±3.12) times per minute and(17.82±3.98) s, which were lower than those of the control group(18.97±3.42) times per minute and(22.16±4.03) s. There were significant differences between the two groups(t=3.702, 3.511, P=0.001, 0.001). Conclusion Radiofrequency ablation with low temperature plasma is effective in the treatment of infantile snoring, which can improve ventilation function and reduce stress response, and is worthy of popularization and application.
