摘要
目的探讨孕期钙营养指导对孕妇及新生儿骨密度和骨代谢水平的影响。方法选取2017年6月至2018年6月在浙江省丽水市妇幼保健院产检的孕妇221例,采用随机数字表法分为观察组(n=109)和对照组(n=112),观察组给予孕期钙营养指导,对照组给予常规孕期保健宣教,检测孕妇、新生儿骨密度及骨代谢水平。结果观察组孕妇分娩后骨密度[(1.02±0.05)g/cm~2]明显高于对照组[(0.93±0.05)g/cm~2],差异有统计学意义(t=13.38,P<0.05);观察组新生儿体质量[(3.80±0.84)kg]和骨密度[(3 010.41±68.87)m/s]明显高于对照组[(3.24±0.80)kg、(2 750.56±60.34)m/s],差异均有统计学意义(t值分别为5.08、29.86,均P<0.05)。观察组新生儿25-羟基维生素D[(19.20±3.10)ng/mL]明显高于对照组[(17.03±2.89)ng/mL],差异有统计学意义(t=5.38,P<0.05);甲状旁腺素[(88.16±21.03)pg/mL]和骨碱性磷酸酶[(180.18±34.29)U/L]均明显低于对照组[(120.03±30.41)pg/mL]、[(240.92±40.04)U/L],差异均有统计学意义(t值分别为-9.04、-12.10,均P<0.05)。结论孕期钙营养指导有助于新生儿骨骼发育,提高骨密度,改善骨代谢水平。
        Objective To investigate the effect of calcium nutrition guidance during pregnancy on bone mineral density and bone metabolism in pregnant woman and newborn. Methods From June 2017 to June 2018, 221 cases of pregnant women in Maternal and Child Health-Care Center of Lishui were selected and randomly divided into observation group(n=109) and control group(n=112) by random number table. Observation group was given calcium nutrition guidance during pregnancy, and control group was given routine pregnancy health education. Bone mineral density and bone metabolism in pregnant women and newborns were detected. Results The bone mineral density of pregnant women in the observation group was significantly higher(1.02±0.05 g/cm~2) after delivery than those in the control group(0.93±0.05 g/cm~2) with statistical significance(t=13.38,P<0.05). The body mass(3.80±0.84 kg) and bone mineral density(3 010.41±68.87 m/s) of newborns in the observation group were significantly higher than those in the control group(3.24±0.80 kg, 2 750.56±60.34 m/s) with statistical significance(t value was 5.08 and 29.86, respectively, both P<0.05). In the observation group, the 25-hydroxyl vitamin D of newborns(19.20±3.10 ng/mL) was significantly higher than that in the control group(17.03±2.89 ng/mL), and the difference was significant(t=5.38,P<0.05). Parathyroid hormone(88.16±21.03 pg/ml) and bone alkaline phosphatase(180.18±34.29 U/L) were significantly lower than the control group(120.03±30.41 pg/mL, 240.92±40.04 U/L), and there were significant differences(t value was-9.04 and-12.10, respectively, both P<0.05). Conclusion Calcium nutrition guidance during pregnancy is beneficial to the development of bone, improvement of bone mineral density, and the improvement of bone metabolism in newborns.
