血管损害危险因素与老年人前列腺体积的相关性分析

摘要
目的探讨血管损害危险因素与良性前列腺增生(BPH)患者前列腺体积(PV)的关系及其影响因素。方法选取在解放军第161医院就诊或体检的军队离退休人员BPH患者366例,根据前列腺体积分为PV≤40 ml组(156例)与PV> 40 ml组(210例),比较两组各血管损害危险因素的差异;采用限制性立方样条模型分析血小板参数与前列腺体积的关系;进一步用多元logistic回归方法分析影响PV的危险因素。结果调整年龄、体质量指数(BMI)、高血压、血糖、总胆固醇和三酰甘油等变量后,限制性立方样条模型显示,血小板分布宽度(PDW)与PV呈剂量-反应关系(P=0.0156);随着年龄的增大,PV增大的风险越高(Ptrend <0.05),≥80岁人群PV增大的风险是<60岁人群的5.088倍(P <0.01);肥胖(BMI≥28 kg/m2)组是BMI正常(BMI <24 kg/m2)组的2.694倍(P=0.03);PDW≥15.90 fL人群是参考组(PDW <15.60 fL)的2.109倍(P=0.021)。结论年龄、肥胖和高水平的PDW是老年PV增大的独立危险因素。
        Objective To Study of correlation between risk factors about vascular lesion and prostate volume in the elderly with benign prostatic hyperplasia(BPH). Methods A total of 366 retired male cadres who were diagnosed with BPH from June,2016 to September,2017 were enrolled in this study. The enrolled subjects were divided into two groups according to the prostate volume(PV ≤ 40 ml and PV > 40 ml). The difference of risk factors about vascular lesion was compared between the two groups. The restricted cubic spline model(RCS) and multivariate logistic regression analysis were used to analyze the association between platelet indices and BPH risk,and identify risk factors affecting the prostatic volume(PV). Results The results from the RCS model showed that there was an association of platelet distribution width(PDW) and the prostatic volume(PV),after adjusting for age,BMI,hypertension,blood glucose,total cholesterol and triglyceride(P for overall association =0.0156,P for no line =0.006);Multivariate logistic regression model indicated that there was a positive association between age and PV among patients with BHP(P < 0.001),and among obese individuals(OR=2.694,95%CI:1.053-6.892) and among the individuals with PDW > 15.90 fL(OR=2.109,95%CI:1.117-3.979). Conclusion Age,obesity and high PDW are independent risk factors for BHP.
