2014～2017年山东省某医院急性胰腺炎患者住院费用及影响因素分析

摘要
目的分析急性胰腺炎患者住院费用构成及影响因素,为合理控制住院费用,减轻患者经济负担提供参考依据。方法收集2014～2017年在山东大学齐鲁医院消化内科住院的526例患者,采用描述性分析、单因素分析及多元线性回归模型等方法进行分析。结果调查526例患者,男性321例(61.00%);女性205例(39.00%)。44岁以下224例(42.60%);其次是45～59岁140人(26.60%);平均住院天数为10 d;平均住院费用是19 118.85元/例。在各单项费用中,西药费占比最高,人均12 986.6元;其次是化验费,人均2 206元。患者的性别和入院途径对住院费用不存在显著差异(P>0.05)。西药费、住院天数、疾病类型是影响患者住院费用的主要因素(P<0.01)。结论西药费是构成急性胰腺炎患者的主要费用项目,西药费、住院天数、疾病类型是影响患者住院费用的主要因素。
Objective To analyze the composition and influencing factors of hospitalization expense among patients with acute pancreatitis,so as to provide the reference for rational control of hospitalization expenses and reduction of patients' economic burden. Methods A total of 526 patients hospitalized in the gastroenterology department of Qilu hospital,Shandong university during 2014-2017 were collected.Descriptive analysis,single-factor analysis and multiple linear regression model were used for analysis. Results A total of 526 patients were surveyed,including 321 males(61.00%) and 205 females(39.00%).The age group of under 44 years old had the highest number of cases(224 cases,42.60%),followed by 45-59 years old(140 cases,26.60%);the average length of stay was 10 d.The average hospitalization cost was 19 118.85 yuan per case.In each cost item,western medicine cost had the highest composition of 12 986.6 yuanper capita;followed by laboratory fee,which was 2 206 yuan per person.There was no significant difference between the patient's gender and admission route in the hospitalization cost(P>0.05).Western medicine expense,hospitalization days and disease type were the main factors affecting hospitalization expense of patients(P<0.01). Conclusion Western medicine expense is the main cost item of acute pancreatitis patients,and western drug cost,hospitalization days and disease type are the main factors affecting hospitalization cost.
