摘要
目的探讨纤维支气管镜肺泡灌洗术对老年慢性阻塞性肺疾病(COPD)合并重症肺部感染患者的临床治疗效果和对血氧指标及预后的影响。方法选择2014年1月-2017年6月医院收治的老年COPD合并重症肺炎患者126例为研究对象,随机分为试验组和对照组,各63例。所有患者均给予常规对症治疗,试验组在此基础上进行纤维支气管镜肺泡灌洗术。对两组患者临床症状消失时间、疗程及对血氧指标[氧分压(PaO2)、二氧化碳分压(PaCO2)、氧合指数(SpO2)、酸碱度(pH)]及预后的影响进行分析,并评价临床疗效。结果试验组咳嗽消失时间、咳痰消失时间、肺部啰音消失时间、抗菌药物使用时间和住院时间分别为(12.71±2.16)、(8.81±2.76)、(5.73±1.25)、(8.54±3.12)和(15.38±3.29)天均短于对照组(P<0.05)。治疗后两组患者PaO2、SpO2均升高,而PaCO2下降,试验组以上指标为(64.28±5.59)mmHg、(95.53±4.35)mmHg、(33.56±5.52)%与对照组比较,差异有统计学意义(P<0.001);两组患者pH差异无统计学意义(P=0.058)。试验组疗效优于对照组(P=0.018)。试验组不良反应和死亡为7.94%(5/63)和4.80%(3/63)与对照组比较差异无统计学意义。试验组真菌定植率为7.94%(5/63)低于对照组20.63%(13/63)(P=0.042)。结论纤维支气管镜肺泡灌洗术治疗老年COPD合并重症肺部感染效果较好,能改善血氧指标和患者预后,具有一定的临床价值。
        OBJECTIVE To investigate the clinical effect of bronchoscopic alveolar lavage on elderly patients with chronic obstructive pulmonary disease(COPD)complicated with severe pulmonary infection and its influence on blood oxygenation index and prognosis.METHODS A total of 126 elderly patients with COPD and severe pneumonia admitted to the hospital from Jan.2014 to Jun.2017 were enrolled in the study.They were randomly divided into the experimental group and the control group,with 63 cases in each group.All patients were given conventional symptomatic treatment,and the experimental group also received bronchoscopic alveolar lavage.The time when clinical symptoms disappeared,the course of treatment and the effects on blood oxygenation indexes(PaO2,PaCO2,SpO2,pH)and prognosis of the two groups were compared and analyzed,and the clinical efficacy was evaluated.RESULTS The time of disappearance of cough,the time of disappearance of cough,the time of disappearance of lung voice,the time of use of antibiotics and the length of hospital stay in the experimental group were(12.71±2.16),(8.81±2.76),(5.73±1.20),(8.54±3.12)and(15.38±3.29),respectively,significantly shorter than those of the control group(P<0.05).After treatment,PaO2 and SpO2 increased in both groups,while PaCO2 decreased.The above indexes in the experimental group were(64.28±5.59)mmHg,(33.56±5.52)%,(95.53±4.35)mmHg,and the difference was significant(P <0.001).The difference in pH between the two groups was significant(P=0.018).The efficacy of the experimental group was significantly better than that of the control group(P<0.05).The incidences of adverse reactions and death in the experimental group were 7.94%(5/63)and 4.80%(3/63),and there was no significant difference from those of the control group.The colonization rate of fungi in the experimental group was 7.94%(5/63),which was significantly lower than that in the control group(20.63%(13/63))(P=0.042).CONCLUSIONBronchoscopic alveolar lavage is a safe and effective treatment for elderly patients with COPD and severe pulmonary infection.It can significantly improve blood oxygenation index and patient prognosis and has certain clinical value.
