儿童气质特点与ADHD共患行为问题的相关性

摘要
目的:探讨儿童气质特点与注意缺陷多动障碍(ADHD)共患行为问题的关系。方法:选取103例符合DSM-Ⅳ诊断标准的ADHD儿童和32例年龄性别匹配的正常对照,并根据Rutter儿童行为问卷将ADHD儿童分为共患行为问题组(77例)及无行为问题组(26例),采用学龄儿童气质量表测查入组儿童气质特点。结果:ADHD共患行为问题儿童气质因子分在活动水平、节律性、适应性、反应强度、心境特点、持久性维度上得分明显高于无行为问题的ADHD儿童(P <0. 01)。ADHD儿童学龄儿童气质量表的心境特点、持久性及反应阈气质因子分与Rutter儿童行为问卷的神经症行为得分呈正相关性(r=0. 66、0. 52、0. 50,均P <0. 05)。结论:ADHD共患行为问题的儿童的气质特点主要表现为活动水平高,生活无节律,适应能力及持久性差,反应强烈、消极。ADHD儿童的气质因子分在心境特点、持久性及反应阈维度上与神经症行为问题可能存在正相关。
        Objective: To explore the correlation between temperament characteristics and behavior problems in children with attention-deficit/hyperactivity disorder(ADHD). Methods: Totally 103 children with ADHD meeting the criterion of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition(DSM-Ⅳ) were selected, and 32 age-and gender-matched normal children were selected as control group. According to the Rutter Children's Behavior Questionnaire, ADHD children were divided into two groups, namely ADHD with behavior problems group(n =77) and ADHD without behavior problem group(n=26). All children were assessed with the Chinese School Children Temperament Scale( CSTS). Results: The ADHD children with behavior problems got higher CSTS scores in activity level, rhythm, adaptability, response intensity, mood characteristics and persistence dimensions than those without behavior problem(Ps <0. 01). The temperament factor scores of CSTS in mood,persistence and reaction dimensions were positively correlated with the scores of neurotic behavior of Rutter Children's Behavior Questionnaire in children with ADHD( r = 0. 66,0. 52,0. 50,Ps < 0. 05). Conclusion: The temperament characteristics of children with ADHD and comorbid behavioral problems are higher activity level, irregular life,poorer adaptability and persistence, stronger reaction and negative mood. The temperament factors of ADHD children may be related to neurobehavioral problems in dimensions of mood,persistence and reaction.
