摘要

目的探讨控制性减压术治疗重型颅脑损伤的临床效果。方法选取我院收治的130例重型颅脑损伤患者为研究对象,根据手术方式的不同将其分为对照组(65例,标准大骨瓣减压术)与观察组(65例,控制性减压术),比较两组患者的治疗效果。结果观察组患者的治疗总有效率高于对照组,并发症总发生率低于对照组(P<0.05);术后1 d及术后3个月,两组患者的颅内压均显著降低,且观察组均低于对照组(P<0.05)。观察组患者大脑中动脉Vmax、Vmin及Vmean水平均高于对照组,RI及PI均低于对照组(P<0.05)。结论控制性减压术治疗重型颅脑损伤疗效显著,值得推广与应用。
        Objective To discuss the clinical effect of controlled decompression on severe craniocerebral injury.Methods A total of 130 patients with severe craniocerebral injury admitted in our hospital were selected as the study objects, and they were divided into control group(65 cases, standard large bone flap decompression) and observation group(65 cases, controlled decompression) according to the different surgical methods. The therapeutic effects of the two groups were compared. Results The total effective rate of treatment in the observation group was higher than that in the control group, and the total incidence of complications was lower than that in the control group(P<0.05). The intracranial pressure of the two groups decreased significantly at 1 day and three months after operation, and those of the observation group were lower than the control group(P <0.05). The levels of Vmax, Vmin and Vmean in the middle cerebral artery of the observation group were higher than those of the control group, and RI and PI were lower than those of the control group(P <0.05). Conclusion Controlled decompression is effective in treating severe craniocerebral injury, which is worthy of promotion and application.
