带膜支架腔内隔绝术联合β受体阻滞剂对Standford B型胸主动脉夹层患者术后生存质量的影响

摘要
目的:探讨带膜支架腔内隔绝术对Standford B型胸主动脉夹层患者术后生存质量的影响。方法:选取2015年2月至2018年1月我院26例StandfordB型胸主动脉夹层患者,均接受带膜支架腔内隔绝术并给予β受体阻滞剂及硝普钠等药物。术后6～12个月进行随访,统计分析本组治疗效果、术前及术后6个月生存质量评分变化情况、并发症发生情况及术后6个月、12个月生存率。结果:本组26例患者均一次性成功植入支架,完全封闭胸主动脉夹层原发破裂口,手术成功率为100.0%(26/26);本组术后社会功能、认知功能、情感功能、角色功能、躯体功能评分均较术前显著增高,差异有统计学意义(P<0.05);本组并发症发生率为23.08%(6/26)、术后6个月、12个月生存率分别为96.15%(25/26)、92.31%(24/26)。结论:采用带膜支架腔内隔绝术联合β受体阻滞剂治疗Standford B型胸主动脉夹层效果显著,手术成功率、术后生存率较高,可有效改善患者生存质量,且安全性较高。
Objective: To explore the effect of endovascular stent-graft exclusion on the quality of life of patients with Standford B thoracic aortic dissection. Methods: From February 2015 to January 2018, 26 patients with Standford B type thoracic aortic dissection were selected in our hospital. All patients underwent endovascular stent-graft exclusion and were given beta-blockers and sodium nitroprusside. Follow-up was carried out 6 to 12 months after operation. The therapeutic effect, the change of quality of life score before and after operation, the occurrence of complications and the survival rate of 6 months and 12 months after operation were statistically analyzed. Results: The26 patients in this group were successfully implanted in one time, completely closed the primary rupture of the thoracic aortic dissection. The success rate was 100.0%(26/26).The postoperative social function, cognitive function, emotional function, The role function and physical function scores were significantly higher than those before surgery(P<0.05). The complication rate of this group was 23.08%(6/26), and the survival rates of 6 months and 12 months after operation were 96.15%(25/26) and 92.31%(24/26)respectively. Conclusion: Endovascular stent-graft exclusion combined with beta-blocker is effective in the treatment of Standford B thoracic aortic dissection. With high success rate, survival rate and high safety, it can effectively improve the quality of life of patients, which is worthy of promotion.
