从《孙文垣医案》浅谈孙一奎治疗痰证学术特色

摘要
《孙文垣医案》是孙一奎诊疗实践的记录,案中夹议夹论,阐发证治,蕴含丰富的临床经验。通过对孙氏三则代表性医案详细分析,借以分析孙氏诊治痰证学术特色。其中积块跳动不止案中,体现孙氏治病求本、怪病责痰的学术特色,治疗中其妙用二陈,对症下药,知常达变,随证易方;脾泄案中,体现孙氏善于析证、圆机活法,且其精研脉诊,急则治标;经闭案中,孙氏去伪存真、补偏救弊,认为气郁痰生、气顺痰消。
Sun Yikui's clinical practice of diagnosis and treatment are recorded in Sun Wenyuan's Medical Cases,which include both medical cases and discussion. This book explains the syndrome and treatment,and contains the abundant clinical experience. By detailed analysis on Sun's three representative cases,Sun Yikui 's academic characteristics in the treatment of phlegm syndrome are analyzed. In the case of beating lump,it reflects that Sun's treatments focus on the root cause of disease and the role of phlegm in rare disease,Erchen Decoction is good use in the treatment,medicine are applied according to indications and modified according to different syndrome. In the case of splenic diarrhea,it reflects that he is good at syndrome differentiation and studies pulse-taking carefully,the proper treatments are selected and modified based on the sufficient theory,and the symptomatic treatments are used in acute condition. In the case of amenorrhoea,he discards the false and retains the true,remedies defects and rectifies errors,it is considers that phlegm is caused by qi stagnation and removed by activating qi.
