摘要
目的分析白内障患者双眼植入一片式衍射型多焦点人工晶体与单眼植入术后视觉质量差异。方法选取2016年7月~2017年9月在我科行白内障超声乳化联合人工晶体植入的患者34例,将双眼植入Tecnis ZMB00多焦点人工晶体患者20例设为双眼多焦组;非主视眼植入多焦点人工晶体、主视眼植入Tecnis ZCB00单焦点人工晶体患者14例设为单眼多焦组。比较两组患者术后3个月双眼裸眼远视力、中间视力、近视力、术后生活质量及脱镜率。结果术后3个月两组患者双眼5 m裸眼远视力及30 cm裸眼近视力比较,差异无统计学意义(P=0.188,P>0.05),60 cm裸眼中间视力单眼多焦组联合组优于双眼多焦组,差异有统计学意义(P=0.028,P<0.05);就夜间生活质量而言,单眼多焦组夜间眩光感觉优于双眼多焦组,差异有统计学意义(P<0.05),两组患者的全程视力的脱镜率均>90%,术后可获得良好的全程视力。结论双眼植入不同的类型的人工晶体同样可以获得良好的双眼单视功能,可以更好提高患者生活质量,尤其是夜间弱照明条件下的生活质量。
Objective To analyze the difference in visual quality between one-eye diffraction multifocal intraocular lens and single eye implantation in cataract patients. Methods From July 2016 to September 2017, 34 patients with phacoemulsification and intraocular lens implantation were enrolled in our department. 20 patients with Tecnis ZMB00 multifocal intraocular lens implantation were set as binocular multifocal group; non-main subject 14 patients with ocular implanted multifocal intraocular lens and primary eye implanted with Tecnis ZCB00 single focus intraocular lens were set as monocular multifocal group.The distant visual acuity, intermediate visual acuity, near visual acuity, quality of life and the rate of lens removal were compared between the two groups 3 months after operation.Results There was no significant difference in the distance between the 5 m naked eye and the 30 cm naked eye near the 3 months after operation(P >0.05). The 60 cm open eye intermediate vision monocular multifocal group was superior. In the binocular multifocal group, the difference was statistically significant(P<0.05). In terms of nighttime quality of life, the monocular multifocal group had better night glare sensation than the binocular multifocal group, the difference was statistically significant( P<0.05), the visual acuity of the whole group of patients was >90%, and good visual acuity was obtained after operation. Conclusion Implantation of different types of intraocular lenses in both eyes can also achieve good binocular single vision function, which can better improve the quality of life of patients,especially the quality of life under weak night illumination conditions.
