摘要
目的探讨早期持续气道正压通气(CPAP)预防早产儿呼吸窘迫综合征(RDS)的效果。方法选择2017年7月-2018年12月我院早产儿病房收治的出生胎龄<30周、日龄<6h早产儿,采用前瞻性随机数字表法将患儿分为治疗组与对照组,治疗组在入院时全部给予CPAP预防治疗,对照组入院时不予CPAP预防,只有发生RDS的患儿才给予CPAP治疗。分析两组患儿RDS发生率、肺泡表面活性物质(PS)使用率、PS再次使用率、机械通气使用率、用氧时间及住院时间。结果最终纳入研究患儿58例,治疗组30例,对照组28例。两组患儿RDS发生率、PS使用率、PS再次使用率、机械通气使用率、用氧时间及住院时间比较,差异均有统计学意义(P<0.05)。结论早期CPAP预防早产儿呼吸窘迫综合征(RDS)的效果确切,可减少患儿RDS发生率、PS使用率、PS再次使用率、机械通气使用率,缩短总的用氧时间及住院时间。
        Objective To explore the clinical curative effect of early use of CPAP for prevent Respiratory distress syndrome in premature infants of <30 weeks gestation. Methods Premature babies of <30 weeks gestation and born <6 hours in our premature babies ward between July 2017 to December 2018 were randomly divided into treatment group and control group. Treatment group was all given CPAP treatment when they hospitalized and the control group was not given CPAP for prevent RDS,only given CPAP after diagnosis RDS. The RDS incidence rate,the PS usage rate,PS reutilization,the usage rate of mechanical ventilation,oxygen therapy time and hospital stays of the two groups of were analyzed. Results fifty-eight infants were included in. There were30 cases of treatment group and 28 cases of control group. There were statistically significant difference in comparison with The RDS incidence rate,the PS usage rate,PS reutilization,the usage rate of mechanical ventilation,oxygen therapy time and hospital stays of the two groups between the two groups(P<0.05). Conclusion Early use of CPAP is effective to prevent Respiratory distress syndrome,It can significantly reduce the RDS incidence rate、the PS usage rate,PS reutilization,the usage rate of mechanical ventilation,reduce oxygen therapy time and hospital stays.
