高尿酸血症和痛风的高危因素分析

摘要
目的探究与分析高尿酸血症及痛风的高危因素相关性分析。方法选取安徽中医药大学第一附属医院自2015年4月~2017年4月收治的90例高尿酸血症患者与90例痛风患者,分别作为高尿酸血症组和痛风组,采用Logistic多因素回归模型分析痛风的发生与肥胖、肝功能、血脂等因素的相关性。结果与高尿酸组比较,痛风组男性患者比例较高,总胆固醇≥5.72 mmol/L、三酰甘油(TG)≥1.71 mmol/L患者比例较高,高密度脂蛋白胆固醇<0.91 mmol/L患者比例较高,低密度脂蛋白胆固醇(LDL-C)≥3.37 mmol/L患者比例较高,载脂蛋白B≥1.20 g/L患者比例较高,谷丙转氨酶≥50 U/L患者比例较高,差异有统计学意义(P <0.05)。两组患者体重指数及载脂蛋白A水平比较,差异无统计学意义(P> 0.05)。将上述因素用Logistic多因素回归分析,可见男性、TG≥1.71 mmol/L、LDL-C≥3.37 mmol/L能够作为引起痛风的高危险因素(OR=2.05、2.45、3.12)。结论男性可作为痛风的主要发病人群,高TG、LDL-C可作为引起痛风的高危因素。
        Objective To explore and analyze the correlation between high risk factors of hyperuricemia and gout.Methods From April 2015 to April 2017, 90 cases of hyperuricemia patients and 90 cases of gout patients admitted to the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine were selected as the hyperuricemia group and the gout group respectively, Logistic multivariate regression model was used to analyze the correlation between the occurrence of gout and obesity, liver function, blood lipid and other factors. Results Compared with the high uric acid group, the gout group had a higher proportion of male patients, higher proportion of patients with total cholesterol ≥5.72 mmol/L and triglycerin(TG) ≥1.71 mmol/L, and higher proportion of patients with high density lipoprotein cholesterol <0.91 mmol/L. The proportion of patients with low-density lipoprotein cholesterol(LDL-C) ≥3.37 mmol/L,apolipoprotein B ≥1.20 g/L and alanine aminotransferase ≥50 U/L was higher, and the difference were statistically significant(P < 0.05). There was no significant difference in BMI and apolipoprotein A levels between the two groups(P > 0.05). The above factors were analyzed by Logistic multivariate regression, and it was found that male, TG ≥1.71 mmol/L, and LDL-C≥3.37 mmol/L could be considered as high risk factors for gout(OR = 2.05, 2.45, 3.12, respectively). Conclusion Men can be the main morbidity group of gout, and high TG and LDL-C can be the high risk factors of gout.
