术前老年耳鼻喉内窥镜检查不良情绪评分与耐痛阈的相关性

摘要
目的:分析术前老年耳鼻喉内窥镜检查不良心理情绪评分与耐痛阈的相关性。方法:回顾性分析2013年7月-2017年2月在我院接受术前耳鼻喉内窥镜检查患者的临床资料,筛选120例患者作为本次研究对象,应用焦虑自评量表(SAS)评价患者焦虑程度,统计术前老年耳鼻喉内窥镜检查患者中不良心理情绪发生率,并对比不同焦虑程度患者耐痛阀,采用Pearson线性相关性分析SAS评分与耐痛阀的相关性。结果:120例行耳鼻喉内窥镜检查的老年患者中,78例患者SAS评分>50分,占比65.00%,其中轻度焦虑30(38.46%)例、中度焦虑23(29.49%)例、重度焦虑25(32.05%)例;以焦虑等级暴露分组,轻度、中度及重度焦虑患者耐痛阀组间差异具有统计学意义(F=44.408,P=0.001),轻度焦虑患者耐痛阀显著高于中度及重度焦虑患者(t=5.491,8.493,P<0.001),中度焦虑患者耐痛阀显著高于重度焦虑(t=3.568,P<0.01);经Pearson相关性分析,SAS分值与耐痛阀具显著负相关(r=-0.413,P<0.001)。结论:术前老年耳鼻喉内窥镜检查不良心理情绪评分与耐痛阀高度负相关,临床因重视对此类患者术前干预,或能改善其不良情绪,避免不良情绪影响耐痛阀。
Objective:To analyze the correlation between negative emotion scores and the pain tolerance threshold in the elderly undergoing(ear,nose and throat)ENT endoscopy before surgery.Methods:The clinical data of patients undergoing ENT endoscopy in our hospital during the period from July 2013 to February 2017 were retrospectively analyzed.A total of 120 patients who met the inclusion and exclusion criteria were selected as the subjects.The degree of anxiety was evaluated by the Self-rating Anxiety Scale(SAS).The incidence of negative emotions was statistically analyzed,and the pain tolerance thresholds of patients with different anxiety levels was compared.The correlation between SAS score and the pain tolerance threshold was analyzed by Pearson linear correlation analysis.Results:In the 120 elderly patients undergoing ENT endoscopy,SAS scores of 78 patients were higher than 50 points,accounting for 65.00%.There were 30(38.46%)cases with mild anxiety,23(29.49%)cases with moderate anxiety and 25(32.05%)cases with severe anxiety.There were significant differences of pain tolerance thresholds between patients with mild,moderate and severe anxiety(F=44.408,P<0.001),patients with mild anxiety had higher pain tolerance than patients with moderate and severe anxiety(t=5.491,8.493;P<0.001),patients with moderate anxiety had higher pain tolerance than patients with severe anxiety(t=3.568,P<0.01).Pearson correlation analysis showed that the SAS score was negatively correlated with the pain tolerance threshold(r=-0.413,P<0.001).Conclusion:The negative emotion score is highly negatively correlated with the pain tolerance threshold in the elderly undergoing ENT endoscopy before surgery.Clinical attention should be paid to the intervention before surgery to avoid negative emotions affecting the pain tolerance threshold.
